摘　　要：目的构建痰培养标本送检前初筛简表并对其预测效果进行验证。方法采用前瞻性观察法,收集2021年2月—11月在某三级甲等综合医院呼吸内科住院的187例患者留取的痰标本及相关资料,构建痰培养标本送检前初筛简表,并对简表的区分度和预测效果进行检验。结果多因素Logistic回归分析结果显示,痰标本粘稠度、标本留取与抗生素使用间隔时间是影响痰培养标本质量的独立危险因素(p<0.05)。痰培养标本送检前初筛简表,ROC曲线下面积为0.724,简表得分为5.5分时,其灵敏度为48.7%,特异度为83.8%。结论痰液粘稠度越高,痰标本留取与抗生素使用间隔时间越长,痰标本培养阳性结果的可能性越大。痰培养标本送检前初筛简表能较好地预测痰培养标本的质量。Objective To construct a brief preliminary screening table for sputum culture specimens before examination and test its predictive effect.Methods Prospective observation method was used to collect the relevant data of 187 sputum specimens from patients hospitalized in respiratory department from February 2021 to November 2021.A brief preliminary screening table was constructed before sputum culture specimens were submitted for examination,and the discrimination and prediction effect of the brief table were tested.Results The results of multivariate logistic regression analysis showed that the viscosity of sputum specimens,the interval between specimen collection and antibiotic use were independent risk factors affecting the quality of sputum culture specimens(p<0.05).The area under the ROC curve was 0.724,the score of the brief table was 5.5,the sensitivity was 48.7%,and the specificity was 83.8%.Conclusion The higher the viscosity of sputum was,the longer the interval between sputum specimen collection and antibiotic use was,and the higher the possibility of positive results of sputum specimen culture would be.The preliminary screening table can predict the quality of sputum culture specimens well.
