内镜下内痔套扎术后应用镇痛泵对手术效果的影响

摘　　要：目的观察内镜下内痔套扎术后应用镇痛泵对手术效果的影响。方法根据术后是否使用镇痛泵,将Ⅲ度内痔300例患者(均采取内镜下内痔套扎术)分为对照组(术后不使用镇痛泵)和试验组(术后使用镇痛泵)。观察两组术后48 h便血、疼痛感及术后1 a内痔复发率等情况。结果两组术前疼痛评分差异无统计学意义(P>0.05),术后均较术前明显下降(P<0.01),且以试验组下降更为显著(P<0.01)。试验组术后便血与内痔复发率均明显低于对照组(P<0.05)。结论内镜下内痔套扎术后应用镇痛泵可减少术后便血发生率和内痔复发率,减轻患者疼痛,值得临床推广。Objective To investigate the impact of the application of analgesic pump after endoscopic hemorrhoid ligation on surgical effect.Methods According to whether postoperative analgesic pump was used,300 patients with Grade III internal hemorrhoids(all receiving endoscopic hemorrhoid ligation)were divided into the Control Group(receiving no postoperative analgesic pump)and the Experimental Group(receiving postoperative analgesic pump).The hematochezia and pain at postoperative 48 h and the recurrence rate of internal hemorrhoids at postoperative 1a were observed.Results There was no significant difference in preoperative pain scores between the two groups(P>0.05),but the preoperative pain scores after operation were significantly decreased compared with those before operation in both groups(P<0.01),and the Experimental Group showed a more significant decrease(P<0.01).After operation,the Experimental Group had the hematochezia and the recurrence rate of internal hemorrhoids significantly lower than the Control Group(P<0.05).Conclusion The application of analgesic pump after endoscopic hemorrhoid ligation can reduce postoperative hematochezia and recurrence rate of internal hemorrhoids,and reduce patients’pain.It is worthy of clinical promotion.
