左旋甲状腺素片治疗妊娠合并亚临床甲状腺功能减退症的效果观察

摘　　要：目的研究左旋甲状腺素片治疗妊娠合并亚临床甲状腺功能减退症的临床效果。方法42例妊娠合并亚临床甲状腺功能减退症患者,均在常规用药基础上采用左旋甲状腺素片治疗。比较患者治疗前后甲状腺功能,分析患者妊娠结局。结果治疗后,患者游离三碘甲状腺原氨酸(FT3)(2.91±1.04)pg/ml和游离甲状腺素(FT4)(0.94±0.26)ng/dl均高于治疗前的(0.87±0.28)pg/ml、(0.26±0.13)ng/dl,促甲状腺素(TSH)(5.23±1.19)μIU/ml低于治疗前的(7.98±2.64)μIU/ml,差异有统计学意义(P<0.05)。所有患者妊娠结局基本良好,出现3例妊娠贫血和4例妊娠期糖尿病患者,未出现自然流产、胎膜早破、早产和宫内窘迫等不良妊娠结局。结论对妊娠合并亚临床甲状腺功能减退症患者,在常规用药基础上采用左旋甲状腺素片进行治疗可以有效改善患者的甲状腺功能,维持良好的妊娠结局,值得推荐。Objective To study the clinical effect of levothyroxine sodium tablets in the treatment of subclinical hypothyroidism during pregnancy.Methods 42 pregnant patients with subclinical hypothyroidism were treated with levothyroxine sodium tablets on the basis of conventional medication.The thyroid function of the patients before and after treatment was compared,and the pregnancy outcomes of the patients were analyzed.Results After treatment,the patients'free triiodothyronine(FT3)(2.91±1.04)pg/ml and free thyroxine(FT4)(0.94±0.26)ng/dl were higher than(0.87±0.28)pg/ml and(0.26±0.13)ng/dl before treatment,and the thyroid stimulating hormone(TSH)(5.23±1.19)μIU/ml was lower than(7.98±2.64)μIU/ml before treatment,and the differences were all statistically significant(P<0.05).The pregnancy outcomes of all patients were basically good,with 3 cases of pregnancy anemia and 4 cases of gestational diabetes mellitus,and no adverse pregnancy outcomes such as spontaneous abortion,premature rupture of membranes,preterm labor and intrauterine distress.Conclusion For pregnant patients with subclinical hypothyroidism,treatment with levothyroxine sodium tablets on the basis of conventional medication can effectively improve the patient's thyroid function and maintain a good pregnancy outcome.It is worth recommending.
