自制可吸引软镜鞘应用于输尿管软镜治疗上尿路结石患者中的效果分析

摘　　要：目的:研究自制可吸引软镜鞘应用于输尿管软镜治疗上尿路结石患者的效果。方法:选取2021年6月—2022年10月收治的80例上尿路结石患者,均行输尿管软镜下碎石术治疗,按随机数字表法分组,对照组(40例)采用无吸引软镜鞘行输尿管软镜碎石术,观察组(40例)采用自制可吸引软镜鞘行输尿管软镜碎石术,对比两组围术期指标、结石清除率及并发症。结果:观察组手术时间、下床活动时间及住院时间较之对照组更短,术中出血量、术后24h VAS评分较之对照组更低,治疗费用较之对照组更少(P<0.05);观察组95.00%(38/40)结石清除率较之对照组75.00%(30/40)更高(P<0.05);观察组2.50%(1/40)并发症发生率较之对照组20.00%(8/40)更低(P<0.05)。结论:自制可吸引软镜鞘应用于输尿管软镜治疗上尿路结石患者,可促进结石清除率提高,减少治疗费用,缩短患者恢复时间,有利于减少并发症。Objective:To study the effect of self-made absorbable soft lens sheath in the treatment of upper urinary tract calculi with ureteroscope.Methods:80 patients with upper urinary tract calculi admitted from June 2021 to June 2022 were selected,all of whom were treated with ureteral soft lens lithotripsy.The patients were divided into two groups according to the random number table.The control group(40 cases)was treated with ureteral soft lens lithotripsy with non suction soft lens sheath,and the observation group(40 cases)was treated with ureteral soft lens lithotripsy with self-made suction soft lens sheath.Perioperative indexes,stone clearance rate,recurrence rate and complications of the two groups were compared.Results:Compared with the control group,the operation time,ambulation time and hospitalization time of the observation group were shorter,the intraoperative blood loss,postoperative 24h VAS score were lower,and the treatment cost was lower in the observation group(P<0.05);The stone clearance rate of 95.00%(38/40)in the observation group was higher than that of 75.00%(30/40)in the control group(P<0.05);The recurrence rate of stone in the observation group(5.00%,2/40)was lower than that in the control group(22.50%,9/40)6 months after operation(P<0.05);The complication rate of 2.50%(1/40)in the observation group was lower than that of 20.00%(8/40)in the control group(P<0.05).Conclusion:The self-made absorbable flexible sheath can be used in the treatment of upper urinary tract calculi with ureteroscope,which can improve the stone clearance rate,reduce the treatment cost,shorten the recovery time of patients,and reduce complications.
