血清β-hCG联合PROG检测对异位妊娠的诊断价值探讨

摘　　要：目的 探讨血清人绒毛膜促性腺激素的β亚单位(β-hCG)联合孕酮(PROG)检测对异位妊娠的诊断价值。方法 选取73例异位妊娠孕妇作为研究组,另选取70例正常宫内妊娠孕妇作为正常组。两组均进行血清β-hCG和PROG检测,比较两组血清β-hCG和PROG水平,血清β-hCG、PROG单独检测及联合检测对异位妊娠的诊断效能。结果 研究组血清β-hCG水平为(1131.65±602.48)IU/L,PROG水平为(18.01±11.27)nmol/L;正常组血清β-hCG水平为(6012.36±1042.55)IU/L, PROG水平为(97.69±10.84)nmol/L。研究组血清β-hCG和PROG水平均显著低于正常组,差异有统计学意义(P<0.05)。血清β-hCG检测异位妊娠的灵敏度为78.08%(57/73),特异度为72.86%(51/70),准确度为75.52%(108/143),阳性预测值为75.00%(57/76),阴性预测值为76.12%(51/67);PROG检测异位妊娠的灵敏度为82.19%(60/73),特异度为75.71%(53/70),准确度为79.02%(113/143),阳性预测值为77.92%(60/77),阴性预测值为80.30%(53/66);血清β-hCG联合PROG检测异位妊娠的灵敏度为95.89%(70/73),特异度为90.00%(63/70),准确度为93.01%(133/143),阳性预测值为90.91%(70/77),阴性预测值为95.45%(63/66)。血清β-hCG联合PROG检测异位妊娠的灵敏度、特异度、准确度、阳性预测值、阴性预测值均显著高于血清β-hCG和PROG单独检测,差异有统计学意义(P<0.05);血清β-hCG单独检测异位妊娠的灵敏度、特异度、准确度、阳性预测值、阴性预测值与PROG单独检测比较,差异无统计学意义(P>0.05)。结论 血清β-hCG联合PROG检测对异位妊娠具有较高的鉴别诊断价值,有利于早期筛查异位妊娠,从而做到早发现早治疗。Objective To discuss the diagnostic value of serum β-human chorionic gonadotropin(β-hCG) combined with progesterone(PROG) detection in ectopic pregnancy. Methods 73 pregnant women with ectopic pregnancy were selected as the research group, and another 70 pregnant women with normal intrauterine pregnancy were selected as the normal group. Serum β-hCG and PROG tests were performed in both groups. Comparison was made on serum β-hCG and PROG levels between the two groups, as well as the diagnostic efficacy of serum β-hCG and PROG detection alone and in combination for ectopic pregnancy.Results The research group had serum β-hCG level of(1131.65±602.48) IU/L, and the PROG level of(18.01±11.27) nmol/L;the normal group had serum β-hCG level of(6012.36±1042.55) IU/L, and the PROG level of(97.69±10.84) nmol/L. The levels of serum β-hCG and PROG in the research group were significantly lower than those in the normal group, and the differences were statistically significant(P<0.05). The sensitivity of serum β-hCG in detecting ectopic pregnancy was 78.08%(57/73), the specificity was 72.86%(51/70), the accuracy was 75.52%(108/143), the positive predictive value was 75.00%(57/76), the negative predictive value was 76.12%(51/67);the sensitivity of PROG in detecting ectopic pregnancy was 82.19%(60/73), the specificity was 75.71%(53/70), the accuracy was 79.02%(113/143), the positive predictive value was 77.92%(60/77),the negative predictive value was 80.30%(53/66);the sensitivity of PROG in detecting ectopic pregnancy was 82.19%(60/73), the specificity was 75.71%(53/70), the accuracy was 79.02%(113/143), the positive predictive value was 77.92%(60/77), the negative predictive value was 80.30%(53/66);the sensitivity of serum β-hCG in combination with PROG was 95.89%(70/73), the specificity was 90.00%(63/70), the accuracy was 93.01%(133/143), the positive predictive value was 90.91%(70/77) and the negative predictive value was 95.45%(63/66).The sensitivity, specificity, accuracy, positive predictive value, and negati
