联合血液灌流、血液透析治疗慢性心肾综合征维持性透析患者顽固性低血压的疗效

摘　　要：目的分析慢性心肾综合征维持性透析患者应用血液灌流与血液透析联合治疗对顽固性低血压的防治效果。方法于2018年3月至2019年11月这一期间,选取本院收治的84例慢性心肾综合征维持性透析患者,都有顽固性低血压出现并随机分为两组,对比组单纯应用血液透析治疗,试验组在血液透析的基础上加用血液灌流治疗,对两组舒张压、收缩压以及顽固性低血压发生率予以比较。结果治疗后试验组收缩压水平高于对比组(P<0.05);试验组治疗后舒张压水平高于对比组(P<0.05);试验组治疗后4周、8周的顽固性低血压发生率均比对比组更低(P<0.05)。结论慢性心肾综合征维持性透析患者应用血液灌流与血液透析联合治疗效果理想,可改善血压水平,减少顽固性低血压的出现。Objective To analyze the preventive and therapeutic effects of hemoperfusion and hemodialysis on refractory hypotension in patients with chronic heart-kidney syndrome undergoing maintenance dialysis.Methods From March 2018 to November 2019,84 patients with chronic cardio-renal syndrome undergoing maintenance dialysis in our hospital were selected.All patients had refractory hypotension and were randomly divided into two groups.The control group was treated with hemodialysis alone,and the experimental group was treated with hemoperfusion on the basis of hemodialysis.The incidence rates of diastolic pressure,systolic pressure and refractory hypotension were compared between the two groups.Results After treatment,systolic blood pressure level in the experimental group was higher than that in the control group(P<0.05).The diastolic blood pressure level in the experimental group was higher than that in the control group(P<0.05).The incidence of refractory hypotension in the experimental group was lower than that in the control group at 4 and 8 weeks after treatment,and the incidence of refractory hypotension in the experimental group was lower than that in the control group(P<0.05).Conclusion The combined therapy of hemoperfusion and hemodialysis is effective for patients with chronic heart-kidney syndrome undergoing maintenance dialysis,which can improve blood pressure level and reduce the occurrence of refractory hypotension.
