改良桡动脉穿刺法在初学者中的应用

摘　　要：目的 观察初学者采用改良法行桡动脉穿刺术的成功率及并发症。方法 选取我院2019年5月至2020年3月手术患者50例,将其随机分为改良组25例和对照组25例,改良组患者行桡动脉穿刺时,针干与皮肤呈10°~25°进针,针蒂见血后即可送管。对照组则针干与皮肤呈30°~45°进行穿刺,穿刺见血后再进1~2 mm,再送管。两种方法均由我院麻醉科住院医师操作。观察两种穿刺法成功率、穿刺针蒂出现动脉血和皮下血肿等发生情况。结果 改良组桡动脉穿刺成功率显著高于对照组(P<0.05);观察组失败率、皮下血肿发生率明显低于对照组(P<0.05);穿刺针蒂出现动脉血发生率稍高于对照组,但差异不显著(P>0.05)。结论 对初学者来说,改良法桡动脉穿刺术成功率高,皮下血肿等并发症发生率低。Objective To observe the success rate and complications of radial artery puncture by improved method for beginners. Methods Fifty cases of surgical patients in our hospital from May 2019 to March 2020 were selected andrandomly divided into improved group(25 cases) and control group(25 cases). In the improved group, the needle was inserted at 10° to 25° with the skin, and the catheter could be delivered after the blood was seen on the needle pedicle. In the control group, the needle was punctured at 30° to 45° to the skin, and then 1-2 mm was inserted into the puncture tube after blood was seen. Both methods were operated by residents of anesthesiology department of our hospital. The success rate of the two puncture methods, arterial blood and subcutaneous hematoma were observed. Results The success rate of radial artery puncture inthe improved group was significantly higher than that in the control group(P<0.05);failure rate and the incidence of subcutaneous hematoma in the improved group was significantly lower than that in the control group(P<0.05);the incidence of arterial blood in the puncture needle pedicle in the improvedgroup was slightly higher than that in the control group(P>0.05). Conclusion For beginners, the improved radial artery puncture has high success rate and low incidence of complications such as subcutaneous hematoma.
