IMB护理联合双重腹水超滤浓缩回输在肝硬化腹水中的应用

摘　　要：目的:分析信息-动机-行为技巧(IMB)护理联合双重腹水超滤浓缩(double ascites ultrafiltration,DUF)回输在肝硬化腹水中的应用。方法:选取2019年8月-2022年8月福建医科大学孟超肝胆医院收治的接受DUF回输治疗的106例肝硬化腹水患者。所有患者给予IMB护理联合DUF回输治疗。比较106例患者治疗前、治疗结束时及治疗后1 d相关指标,分析106例患者其他指标及住院期间临床疗效。结果:DUF回输治疗结束时血氧饱和度高于治疗前,治疗后1 d,106例患者舒张压及空腹血糖均低于治疗前,腹围小于治疗前,24 h尿量多于治疗前(P<0.05)。一次超滤液量3350.00(2200.00,4700.00)ml,细胞浓缩液量1325.00(900.00,1700.00)ml,平均治疗时间为(1.7±0.3)h。在住院治疗期间,106例患者中有效96例,无效8例,进展2例,有效率高达90.6%。结论:IMB护理联合DUF回输治疗能够促进腹水消退,改善患者腹围及24 h尿量,值得临床推广应用。Objective:To analyze the application of information motivation behavioral skills model(IMB)nursing combined with double ascites ultrafiltration concentration(DUF)and reinfusion in ascites of liver cirrhosis.Method:A total of 106 patients with cirrhotic ascites who received DUF reinfusion treatment admitted to Mengchao Hepatobiliary Hospital of Fujian Medical University from August 2019 to August 2022 were selected.All patients were given the IMB nursing combined with DUF reinfusion treatment.The relevant indexes of 106 patients before treatment,at the end of treatment,and 1 day after treatment were compared,and other indexes of 106 patients and clinical efficacy during hospitalization were analyzed.Result:At the end of DUF reinfusion treatment,the blood oxygen saturation was higher than that before treatment.One day after treatment,the diastolic pressure and fasting blood glucose of 106 patients were lower than those before treatment,the abdominal circumference was lower than that before treatment,and the urine volume in 24 hours was higher than that before treatment(P<0.05).The amount of ultrafiltration solution was 3350.00(2200.00,4700.00)ml,the amount of cell concentrate was 1325.00(900.00,1700.00)ml,and the average treatment time was(1.7±0.3)h.During hospitalization,96 of 106patients were effective,8 were ineffective,and 2 were progressive.The effective rate was as high as 90.6%.Conclusion:IMB nursing combined with DUF reinfusion treatment can promote ascites regression,and improve patients’abdominal circumference and 24 hour urine volume,which is worthy of clinical promotion and application.
