股方肌骨瓣移植联合空心加压螺纹钉内固定治疗非骨化性纤维瘤头下型的临床效果探究

摘　　要：目的:分析股方肌骨瓣移植联合空心加压螺纹钉内固定治疗非骨化性纤维瘤(non-ossifying fibroma,NOF)头下型的效果。方法:选择2017年1月-2020年11月130例福清向阳骨科医院收治的NOF头下型患者作为研究对象,按照随机数字表法分为试验组与对照组,各65例。对照组给予空心加压螺纹钉内固定治疗,试验组给予股方肌骨瓣移植联合空心加压螺纹钉内固定治疗。对比两组疗效、骨代谢指标与Harris评分。结果:试验组手术时间、术后骨折愈合时间均优于对照组,手术出血量少于对照组(P<0.05)。试验组术后3个月骨折愈合情况优于对照组(P<0.05)。试验组术后3个月优良率、Harris评分及骨保护素(osteoprotegerin,OPG)、血清碱性磷酸酶(alkaline phosphatase,ALP)及骨钙素(bone gla protein,BGP)水平均高于对照组,差异均有统计学意义(P<0.05)。结论:股方肌骨瓣移植联合空心加压螺钉治疗NOF头下型患者效果显著,可有效促进术后骨折愈合及髋关节功能恢复,具临床推广意义。Objective:To analyze the efficacy of quadratus femoris bone flap graft combined with hollow pressurized screw internal fixation in the treatment of non-ossifying fibroma(NOF)subcephalic type.Method:A total of 130 patients with NOF subcephalic type in Fuqing Xiangyang Orthopedic Hospital from January 2017 to November 2020 were selected as the study subjects,they were divided into the experimental group and the control group according to the random number table method,65 cases in each group.The control group was treated with hollow pressurized screw internal fixation,and the experimental group was treated with quadratus femoris bone flap graft combined with hollow pressurized screw internal fixation.The efficacy,bone metabolism index and Harris score were compared between two groups.Result:The operative time and postoperative fracture healing time of the experimental group were better than those of control group,and the amount of surgical bleeding was less than that of control group(P<0.05).The fracture healing in the experimental group at 3 months after surgery was better than that in the control group(P<0.05).The excellent and good rate,Harris score and osteoprotegerin(OPG),alkaline phosphatase(ALP)and bone gla protein(BGP)levels of the experimental groups at 3 months after surgery were higher than those of control group,the differences were statistically significant(P<0.05).Conclusion:The effect of quadratus femoris bone flap graft combined with hollow pressurized screw internal fixation in the treatment of NOF subcephalic type is effective,it can effectively promote the fracture healing and hip joint function recovery after operation,and has clinical significance.
