雌二醇片/雌二醇地屈孕酮片联合黄连温胆汤加味治疗湿热痰扰型围绝经期综合征伴抑郁的效果

摘　　要：目的:探究雌二醇片/雌二醇地屈孕酮片联合黄连温胆汤加味治疗湿热痰扰型围绝经期综合征伴抑郁患者的效果。方法:于2018年3月-2021年5月在莆田市第一医院进行治疗的湿热痰扰型围绝经期综合征伴抑郁患者中随机选取94例,采用简单化随机分组法分为观察组和对照组,各47例。对照组予以雌二醇片/雌二醇地屈孕酮片等进行治疗,观察组则以对照组治疗方案为基础加用黄连温胆汤加味。比较两组治疗3个月后临床效果,治疗前及治疗3个月后中医症候积分、血清相关因子[5-羟色胺(5-HT)、雌二醇(E_(2))、卵泡刺激素(FSH)],治疗前及治疗1、3个月后抑郁状况[汉密尔顿抑郁量表(HAMD)]。结果:观察组治疗3个月后临床效果高于对照组(P<0.05)。治疗前,两组中医症候积分、血清相关因子比较,差异均无统计学意义(P>0.05);治疗3个月后,两组心烦不寐、口苦痰多、头晕目眩、烦躁焦虑评分及FSH水平较治疗前均下降,且观察组均低于对照组,两组5-HT、E_(2)水平较治疗前均升高,且观察组均高于对照组(P<0.05)。治疗前,两组抑郁状况比较,差异均无统计学意义(P>0.05);两组治疗1、3个月后HAMD量表评分较治疗前均下降,且两组治疗3个月后HAMD量表评分均低于治疗1个月后(P<0.05),观察组均低于同时间段对照组(P<0.05)。结论:雌二醇片/雌二醇地屈孕酮片联合黄连温胆汤加味治疗湿热痰扰型围绝经期综合征伴抑郁具有可观的治疗效果,还可帮助患者改善临床症状,调节激素水平,缓解抑郁病情,具有临床价值。Objective:To explore the effect of Estradiol Tablets/Estradiol Didroxyprogesterone Tablets combined with modified Huanglian Wendan Decoction in the treatment of perimenopausal syndrome with depression of dampness-heat phlegm disturbance type.Method:From March 2018 to May 2021,94 cases of perimenopausal syndrome with depression of dampness-heat phlegm disturbance type who treated in Putian First Hospital were randomly selected and divided into the observation group and the control group by simplified random grouping method,47 cases in each group.The patients in the control group were treated with Estradiol Tablets/Estradiol Didroxyprogesterone Tablets,and the patients in the observation group were treated with modified Huanglian Wendan Decoction based on the treatment scheme of the control group.The therapeutic effects of two groups after 3 months of treatment,the scores of TCM symptoms and serum related factors[5-hydroxytryptamine(5-HT),estradiol(E_(2)),follicle stimulating hormone(FSH)]before treatment and after 3 months of treatment were compared between two groups,the depression status[Hamilton depression rating scale(HAMD)]of two groups before treatment and after 1,3 months of treatment were compared.Result:The clinical effect of the observation group after 3 months of treatment was higher than that of the control group(P<0.05).Before treatment,there were no significant differences in TCM symptom scores and serum related factors between two groups(P>0.05);after 3 months of treatment,the scores of restlessness and sleeplessness,bitterness and phlegm,dizziness,irritability and anxiety,and FSH levels in two groups were lower than those before treatment,the observation group were lower than those in the control group,the levels of 5-HT and E_(2) in two groups were higher than those before treatment,and and the levels of 5-HT and E_(2) in the observation group were higher than those in the control group,and the observation group were higher than those in the control group(P<0.05).Before treatment,there was no sign
