基于CTT与IRT的糖尿病生命质量量表QLICD-DM(V2.0)条目分析及评价

摘　　要：目的应用经典测量理论(CTT)和项目反应理论(IRT)对第2版糖尿病患者生命质量量表QLICD-DM(V2.0)进行条目分析和评价。方法采用QLICD-DM(V2.0)量表对242例糖尿病患者进行自评式调查。采用CTT中的变异度法、相关系数法、因子分析法、克朗巴赫α系数法对量表条目质量进行分析,采用IRT分析软件Multilog7.03分析量表条目的难度系数、区分度系数和信息量。结果CTT结果显示,QLICD-DM(V2.0)量表的42个条目中,除4个条目[GPH3(影响性功能)、GPH4(大便正常)、GPS3(生活有乐趣)、GPS10(乐观看待疾病)]需改进之外,其他38个条目均有较好的特性。IRT结果显示,各条目的信息量均>0.119(5/42),条目的区分度分布在0.99~1.45;36个条目的难度系数分布在[-4,4],随难度等级增加而呈现出单调递增趋势。有6个条目(GPH1、GPH6、GPH8、GS02、GS03、GS04)还需进一步完善。结论经CTT及IRT方法分析显示,QLICD-DM(V2.0)量表的大部分条目具有良好的特性,但仍有少部分条目需要进一步完善。Objective To analyze and evaluate the items of the Quality of Life Instruments for Chronic Diseases-Diabetes Mellitus[QLICD-DM(V2.0)]based on classical test theory(CTT)and item response theory(IRT).Methods The scale QLICD-DM(V2.0)was used for a self-evaluation survey in 242 patients with diabetes mellitus.The data was analyzed by four statistical methods based on CTT including methods of variation,correlation coefficients,factor analysis and Cronbachαcoefficients.The difficulty,differentiation coefficients and information amount of items were analyzed by IRT software Multilog 7.03.Results The results based on CTT showed that among 42 items,except for 4 items of GPH3,GPH4,GPS3,and GPS10 needing improvements,the other 38 items had good features.IRT results showed that the information amount of each item was greater than 0.119(5/42),and the differentiation of items was distributed in the range of 0.99–1.45.The difficulty coefficient of 36 items was distributed in the range of[-4,4],which showed a monotonically increasing trend with the increase of difficulty levels.However,6 items(GPH1,GPH6,GPH8,GS02,GS03 and GS04)still needed to be further improved.Conclusion CTT and IRT show that most items of the scale QLICD-DM(V2.0)have good characteristics,but some items still need to be further improved.
