摘  要:转诊合作是解决医疗保健系统中利用率失衡的主要方法之一,合作过程中通常涉及到医院何时需要转诊,转诊哪些疾病类型的患者,以及对应疾病应转诊多少患者等决策问题。因此建立了针对床位数的阈值控制仿真模型帮助医院做出转诊决策。为求解上述模型,结合粒子群算法(PSO)和最优计算量分配方法(OCBA)提出了PSO-OCBA的仿真优化算法。仿真结果表明,所提出的仿真优化算法能够高效求解出最优控制阈值。阈值控制方法能够帮助医院针对各类疾病做出实时转诊决策,并能有效平衡系统中医院资源利用率。Referral cooperation is one of the main methods to solve the imbalance of utilization rate in the medical care system.The process of cooperation usually involves decision-making issues such as when the hospital needs to implement referrals,which types of patients should be referred,and how many corresponding patients should be referred.In order to solve these problems,this paper establishes a threshold control simulation model to help hospitals make decisions.To solve the model,a PSO-OCBA simulation optimization algorithm is also proposed in combination with Particle swarm algorithm(PSO)and Optimal Computational Budget Allocation(OCBA).Finally,the numerical experiment results show that the proposed algorithm can efficiently solve the optimal control threshold for the referral decision problem.The threshold control method also helps hospitals make referral decisions for each disease type in real time and effectively balances the utilization rates of hospital resources in the system.
