不同病情程度精索静脉曲张患者的超声参数和血清FSH水平及影响因素分析

摘　　要：目的探讨不同病情程度精索静脉曲张(VC)患者超声参数、血清卵泡刺激素(FSH)水平及影响因素。方法选取我院VC患者98例为研究组,另选同期健康体检者98例为对照组,均行超声检查其平静呼吸时精索内静脉内径(DR)、反流持续时间(TR),并抽取血液样本测FSH水平;比较研究组与对照组、研究组不同病情程度、不同亚型(单纯VC、VC伴不育)患者上述指标的差异,Logistic回归分析VC病情的影响因素。结果研究组DR、TR、FSH水平均高于对照组,差异均有统计学意义(P均<0.05);不同病情程度VC患者在年龄、高血压病史、吸烟情况、DR、TR、FSH水平、不育比例方面比较,差异均有统计学意义(P均<0.05);VC不育患者DR、TR、血清FSH水平均高于单纯VC患者,差异均有统计学意义(P均<0.05)。Logistic回归分析表明,年龄、高血压病史、吸烟情况、DR、TR、FSH均为VC病情影响因素(P均<0.05)。结论超声参数(DR、TR)及血清FSH水平与精索静脉曲张病情程度密切相关,二者联合应用可从形态学及分子生物学层面为临床评价VC病情程度提供参考,有助于指导临床完善治疗方案。Objective To explore the ultrasound parameters,serum follicle stimulating hormone(FSH)levels and influencing factors in patients with varicocele(VC)of different disease severity.Methods 98 patients with VC in our hospital were selected as the study group,and 98 healthy subjects during the same period were selected as the control group.All of them underwent ultrasound examination of internal spermatic vein diameter during resting respiration(DR)and reflux duration(TR),and blood samples were taken for measurement FSH level.Compare the difference of the above indicators between the study group and the control group,the study group with different disease severity and different subtypes(simple VC,VC with infertility),and Logistic regression analysis of the influencing factors of VC.Results The levels of DR,TR,and FSH in the study group were higher than those in the control group,and the differences were statistically significant(all P<0.05);There were statistically significant differences in age,history of hypertension,smoking status,DR,TR,FSH levels and infertility ratio among VC patients with different disease severity(all P<0.05);the DR,TR,and serum FSH levels of VC infertile patients were higher than those of simple VC,and the differences were statistically significant(all P<0.05).Logistic regression analysis showed that age,history of hypertension,smoking status,DR,TR,FSH are all influencing factors of VC condition(all P<0.05).Conclusion Ultrasound parameters(DR,TR)and serum FSH levels are closely related to the degree of varicocele.Combined use of these methods can provide a reference for clinical evaluation of VC from morphology and molecular biology,and help guide the clinical improvement of the treatment plan.
