益肾通淋排石汤联合体外震波碎石治疗泌尿系统结石临床研究  

Clinical Study on Yishen Tonglin Paishi Tang Combined with Extracorporeal Shock Wave Lithotripsy for Urinary Tract Calculi

摘　　要：目的:观察益肾通淋排石汤联合体外震波碎石治疗泌尿系统结石(UTC)的临床疗效。方法:选择80例UTC患者,以随机数字表法分为治疗组与对照组各40例。对照组给予体外震波碎石治疗,治疗组在体外震波碎石术后给予益肾通淋排石汤治疗。比较2组排石效果、并发症发生率,以及治疗前后中医证候评分、炎症因子水平。结果:治疗组结石排尽率97.50%,高于对照组77.50%(P<0.05)。治疗组结石排尽时间(5.04±0.45)d,短于对照组(6.55±0.42)d (P<0.05)。治疗后,2组中医证候积分均较治疗前降低(P<0.05),治疗组中医证候积分低于对照组(P<0.05)。治疗后,2组血清降钙素原(PCT)、C-反应蛋白(CRP)水平均较治疗前降低(P<0.05),治疗组血清PCT、CRP水平均低于对照组(P<0.05)。治疗组并发症发生率2.50%,低于对照组15.00%(P<0.05)。结论:益肾通淋排石汤联合体外震波碎石治疗UTC,可改善患者的症状、体征,减轻机体炎症反应,提高排石效果,减少并发症发生。Objective: To observe the clinical effect of the therapy of Yishen Tonglin Paishi Tang combined with extracorporeal shock wave lithotripsy on urinary tract calculi(UTC). Methods: A total of80 patients with UTC were selected and divided into the treatment group and the control group according to the random number table method, with 40 cases in each group. The control group was treated with extracorporeal shock wave lithotripsy,and the treatment group was treated with Yishen Tonglin Paishi Tang after extracorporeal shock wave lithotripsy. The stone removal effect,incidence of complication,traditional Chinese medicine(TCM) syndrome score and levels of inflammatory factors before and after treatment were compared between the two groups. Results:The stone drainage rate in the treatment group was 97.50%,higher than that of 77.50% in the control group(P<0.05). The stone discharge time in the treatment group was(5.04 ± 0.45) days, shorter than that of(6.55 ± 0.42) days in the control group(P<0.05). After treatment, the TCM syndrome scores in the two groups were decreased when compared with those before treatment(P<0.05),and the TCM syndrome score in the treatment group was lower than that in the control group(P<0.05). After treatment,the levels of serum procalcitonin(PCT) and C-reactive protein(CRP)in the two groups were decreased when compared with those before treatment(P<0.05),and the levels of serum PCT and CRP in the treatment group were lower than those in the control group(P<0.05). The incidence of complications in the treatment group was 2.50%,lower than that of 15.00% in the control group(P<0.05). Conclusion:The therapy of Yishen Tonglin Paishi Tang combined with extracorporeal shock wave lithotripsy can improve the symptoms and signs of UTC patients, reduce the body’s inflammatory responses,improve the effect of stone removal,and reduce the incidence of complications.
