放射性核素99mTc-DTPA显像在诊断腹膜透析并发胸腹漏中的应用研究

摘　　要：目的 通过利用放射性核素自身显像的特点,研究放射性核素锝-标记亚锡喷替酸(Tc-DTPA)与透析液混合滴注腹腔显像在PD患者可疑胸腹漏诊断中的应用。方法 10例因肾功能衰竭行腹膜透析治疗的患者透析后出现胸腔积液,将Tc-DTPA与2L 1.5%葡萄糖腹膜透析液混合,行放射性核素Tc-DTPA静态及延迟显像,观察是否存在胸腹漏。结论 10例可疑胸腹漏患者中,6例经局部静态显像及延迟显像,在15 min~8 h内胸腔出现异常放射性增高影,诊断胸腹漏;4例经过局部静态显像及延迟显像至6~8 h,均未见胸腔内出现异常放射性分布,显像阴性,排除胸腹漏。结论 放射性核素Tc-DTPA显像对可疑胸腹漏诊断阳性率高,对进一步指导治疗具有重要应用价值,值得临床推广。Objective To study the application of technetiumTc-labeled stannic acid(99 mTc-DTPA) and dialytic fluid in the diagnosis of suspected thoracoabdominal leakage in PD patients by using the characteristics of radionuclide auto-imaging.Methods Pleural effusion occurred in 10 patients undergoing PD treatment for renal failure after dialysis.Tc-DTPA was mixed with 2 L 1.5% dextran peritoneal dialysis fluid, and static and delayed radionuclideTc-DTPA imaging was performed to observe whether thoracoabdominal leakage was present.Results Six of the 10 patients with suspected thoracoabdominal leakage were diagnosed by local static imaging and delayed imaging with abnormal radiation enhancement in the thoracic cavity within 15 min to 8 h. Four patients underwent local static imaging and delayed imaging until 6~8 h, and no abnormal radioactivity distribution was found in the thoracic cavity. The imaging was negative and thoracoabdominal leakage was ruled out.Conclusion RadionuclideTc-DTPA imaging has a high positive rate in the diagnosis of suspicious thoracoabdominal leakages. It has important application value for further treatment and is worthy of clinical promotion.
