基于真实世界的中医临床科研大数据平台研发现状、不足及展望

摘　　要：真实世界中医临床科研范式的核心是临床科研一体化。中医临床一体化平台首先被开发出来,不同的平台或侧重科研或侧重辅助医院日常工作,随着时代发展和科技进步,中医临床科研大数据平台的概念被提出。目前,已有关于建设中医临床科研大数据平台的技术方法和架构设计的探讨。现有的平台或构想存在一定不足,基于此我们提出对中医临床科研大数据平台的构想和展望,平台以人工智能为技术支持、真实世界为数据管理方式、单病例随机对照试验为临床研究方法,可处理大量、来源不同的数据,同时可辅助临床决策,这样的平台可兼顾中医药循证医学特点和个体精准施治要求,既满足科研需求又能辅助临床决策。The core of clinical research paradigm of TCM(Traditional Chinese Medicine) in the real world is the integration of clinical research. TCM clinical integration platform was first developed. Different platforms either focus on scientific research or assist hospital daily work. With the development of the Times and technological progress, the concept of TCM clinical research big data platform was put forward. At present, there have been discussions on the technical methods and architecture design of the construction of platform. The main shortcomings of the existing platforms or ideas are "difficult to support prospective clinical research", "limited volume of data that can be accommodated", "incomplete collection of clinical information" and "unable to support prediction of disease outcome". To supplement the above deficiencies, the TCM clinical research big data platform needs to take artificial intelligence as the technical support, Real-World Study as the data management mode, and single case randomized controlled trial as the clinical research method. Such a platform can take into account the characteristics of TCM evidence-based medicine and the requirements of individual precise treatment, which can not only meet the needs of scientific research, but also assist clinical decision-making.
