环磷酰胺静脉冲击+糖皮质激素治疗特发性膜性肾病的效果及安全性分析

摘　　要：目的 探究环磷酰胺静脉冲击+糖皮质激素治疗特发性膜性肾病的临床效果及安全性。方法 78例特发性膜性肾病患者,采用随机数字表法分为对照组和研究组,各39例。对照组应用糖皮质激素治疗,研究组应用环磷酰胺静脉冲击+糖皮质激素治疗。比较两组治疗效果、不良反应发生情况及治疗前后实验室指标(24 h尿蛋白定量、血清白蛋白)。结果 研究组总有效率92.31%高于对照组的74.36%,差异有统计学意义(P<0.05)。研究组不良反应发生率为12.82%,与对照组的7.69%比较,差异无统计学意义(P>0.05)。治疗前,两组24 h尿蛋白定量、血清白蛋白水平比较,差异无统计学意义(P>0.05);治疗后,研究组24 h尿蛋白定量(2.14±0.42)g/24 h低于对照组的(3.28±1.08)g/24 h,血清白蛋白(36.29±5.31)g/L高于对照组的(31.86±6.76)g/L,差异有统计学意义(P<0.05)。结论 环磷酰胺静脉冲击+糖皮质激素治疗特发性膜性肾病疗效显著,不良反应少,安全性高,可广泛推广应用。Objective To investigate the clinical effect and safety of intravenous cyclophosphamide pulse plus glucocorticoid in the treatment of idiopathic membranous nephropathy.Methods A total of 78 patients with idiopathic membranous nephropathy were divided into a control group and a research group according to random numerical table,with 39 patients in each group.The control group was treated with glucocorticoids,and the research group was treated with intravenous cyclophosphamide pulse + glucocorticoids.The therapeutic effect,occurrence of adverse reactions and the laboratory indexes(24 h urinary protein quantification and serum albumin) before and after treatment were compared between the two groups.Results The research group had higher total effective rate of 92.31% than 74.36% in the control group,and the difference was statistically significant(P0.05).Before treatment,there was no statistically significant difference in 24 h urinary protein quantification and serum albumin level between the two groups(P>0.05).After treatment,the research group had lower 24 h urinary protein quantification of(2.14±0.42) g/24 h than(3.28±1.08) g/24 h in the control group,and higher serum albumin of(36.29±5.31) g/L than(31.86±6.76) g/L in the control group,and the differences were statistically significant(P<0.05).Conclusion Intravenous cyclophosphamide pulse + glucocorticoid in the treatment of idiopathic membranous nephropathy has significant efficacy,less adverse reactions and high safety,which can be widely promoted and applied.
