曲安奈德鼻喷雾剂与克拉霉素联合鼻内镜手术治疗慢性鼻窦炎伴鼻息肉的效果分析

摘　　要：目的 分析曲安奈德鼻喷雾剂与克拉霉素联合鼻内镜手术治疗慢性鼻窦炎伴鼻息肉的效果。方法 150例慢性鼻窦炎伴鼻息肉患者,随机分为对照组(74例)及研究组(76例)。对照组单独进行鼻内镜手术治疗,研究组采用曲安奈德鼻喷雾剂与克拉霉素联合鼻内镜手术治疗。比较两组临床疗效,病情恢复指标(术后鼻腔通气时间、术后鼻黏膜恢复时间及鼻窦CT评分),复发情况。结果 研究组患者的总有效率96.05%高于对照组的79.73%,差异具有统计学意义(P<0.05)。研究组术后鼻腔通气时间(4.07±1.03)d、术后鼻黏膜恢复时间(3.55±0.87)d短于对照组的(4.95±0.97)、(4.11±0.93)d,差异具有统计学意义(P<0.05)。研究组术后3个月的鼻窦CT评分(1.51±0.17)分低于对照组的(2.25±0.27)分,差异具有统计学意义(P<0.05)。进行为期6个月的随访观察,研究组复发率为1.32%(1/76),低于对照组的8.11%(6/74),差异具有统计学意义(P<0.05)。结论 对慢性鼻窦炎伴鼻息肉患者采用曲安奈德鼻喷雾剂与克拉霉素联合鼻内镜手术治疗,可取得令人满意的近远期效果,值得宣传、推广。Objective To analyze the effect of triamcinolone acetonide nasal spray and clarithromycin combined with endoscopic sinus surgery in the treatment of chronic sinusitis with nasal polyps.Methods A total of 150 patients with chronic sinusitis and nasal polyps were randomly divided into a control group(74 patients)and a research group(76 patients).The control group was treated by nasal endoscopic surgery alone,while the research group was treated by triamcinolone acetonide nasal spray and clarithromycin combined with nasal endoscopic surgery.The clinical efficacy,recovery indexes(postoperative nasal ventilation time,postoperative nasal mucosa recovery time and sinus CT score),and recurrence were compared between the two groups.Results The total effective rate in the research group was 96.05%,which was higher than 79.73% in the control group,and the difference was statistically significant(P<0.05).The postoperative nasal ventilation time(4.07±1.03) d and postoperative nasal mucosa recovery time(3.55±0.87) d in the research group were shorter than(4.95±0.97)and(4.11±0.93) d in the control group,and the differences were statistically significant(P<0.05).3 months after surgery,the sinus CT score in the research group was(1.51±0.17) points,which was significantly lower than(2.25±0.27) points in the control group,and the difference was statistically significant(P<0.05).Follow up observation for 6 months showed that the recurrence rate in the research group was 1.32%(1/76),which was lower than 8.11%(6/74) in the control group,and the difference was statistically significant(P<0.05).Conclusion Triamcinolone acetonide nasal spray and clarithromycin combined with endoscopic sinus surgery can achieve satisfactory short-term and long-term results,which is worthy of publicity and promotion.
