支气管镜集中清洗消毒防控医院感染的效果评估

摘　　要：目的 评估支气管镜集中清洗消毒预防医院感染的效果。方法 选取科室分散清洗消毒使用后的纤维支气管镜175例次为A组,集中清洗消毒使用后的纤维支气管镜202例次为B组。另纳入同一时间段采用A组或B组支气管镜检查的收治的患者作为研究对象,其中将采用A组支气管镜检查的患者做为对照组(n=425),采用B组支气管镜检查的患者做为观察组(n=474)。结果 B组清洗消毒后目测法合格率、蛋白残留测定合格率高于A组(p<0.05);B组清洗消毒后管腔采样法和外表采样法细菌培养结果均优于A组(p<0.05);观察组医院感染发生率低于对照组(p<0.05);治疗后观察组体温、WBC、PCT、hs-CRP、PaO、PaCO较前无明显变化(p>0.05),治疗后对照组WBC、PCT、hs-CRP、PaCO水平显著降低,PaO水平显著升高(p<0.05)。结论 消毒供应中心集中供应纤维支气管镜清洗消毒管理可有效提高清洗和消毒质量,降低医院感染风险。Objective To evaluate the effect of centralized bronchoscopic cleaning and disinfection management in the prevention of nosocomial infections.Methods 175 cases of fiber bronchoscopy after decentralized cleaning and disinfection were selected as group A,and 202 cases after centralized cleaning and disinfection were selected as group B.In addition,patients who received bronchoscopy in group A or group B during the same period were included as the research objects.Patients receiving bronchoscopy in group A were selected as the control group (n=425),and those receiving bronchoscopy in group B were selected as the observation group (n=474).Results The qualified rate of visual inspection and protein residue determination after cleaning and disinfection in group B was higher than that in group A (p<0.05).The results of luminal sampling and surface sampling in group B were better than those in group A after cleaning and disinfection (p<0.05).The incidence of nosocomial infection in observation group was lower than that in control group (p<0.05) After treatment,the body temperature,WBC,PCT,hs-CRP,PaOand PaCOin the observation group had no significant changes compared with before(p>0.05).After treatment,the levels of WBC,PCT,hs-CRP and PaCOin the control group were significantly decreased,while the level of PaOwas significantly increased (p<0.05).Conclusion The centralized supply of bronchoscopic cleaning and disinfection management in the disinfection supply center can effectively improve the quality of cleaning and disinfection and reduce the risk of nosocomial infection.
