针刀松解联合三七活骨丸治疗气滞血瘀型酒精性骨股头坏死的临床观察

摘　　要：目的:观察针刀松解结合三七活骨丸医治酒精性股骨头坏死(AOFH)的诊疗效果。方法:将不愿接受手术或缺乏经济条件的70例AOFH患者随机分为治疗组和对照组,每组35例,针刀松解作为两组病患的基础治疗手段,治疗组在基础治疗的同时内服三七活骨丸,6 g/丸,1丸/次,3次/d口服,共30 d,两组治疗前后均进行Harris、VAS、SF-36和中医证候评分,综合计算评估两组的临床疗效,计算有效率。结果:治疗前两组的Harris及SF-36评分低于治疗后,且对照组的相关评分也低于治疗组,差异有统计学意义(P<0.05);治疗前两组的VAS及中医证候评分均高于治疗后,且对照组相关评分高于治疗组,差异有统计学意义(P<0.05)。综合相关量表评分以及临床表现,治疗组的总有效率为94%(32/34),对照组的总有效率为81%(27/33),两组比较差异有统计学意义(P<0.05)。结论:对于不愿接受手术医治及缺乏经济条件的AOFH患者,在针刀松解的治疗基础上辅以三七活骨丸治疗疗效较好。Objective: To observe the therapeutic effect of acupotomy decompression combined with Sanqi Huogu Pill in treatment of alcohol-induced osteonecrosis of femoral head(AOFH). Methods: 70 AOFH patients were randomly divided into the treatment group(n=35) and the control group(n=35). Acupotomy decompression was used as the basic treatment for both groups, and the treatment group was also treated with Sanqi Huogu Pill, 6 g/pill, 1 pill/time, 3 times a day for 30 days. The scores of Harris, VAS, SF-36 and TCM symptoms were performed before and after the treatment in both groups, and the clinical efficacy was comprehensively evaluated in the two groups. Results: Harris score and SF-36 score were lower before the treatment than those after the treatment in the two groups, and Harris score and SF-36 score were lower in the control group than those in the treatment group after the treatment(P<0.05). VAS score and TCM symptoms score were higher before treatment than those after the treatment in the two groups, and VAS score and TCM symptoms score were higher in the control group than those in the treatment group after the treatment(P<0.05). The effective rate was 94%(32/34) in the treatment group, which was significantly higher than 81%(27/33) in the control group(P<0.05). Conclusion: Acupotomy decompression combined with Sanqi Huogu Pill has a better therapeutic effect in treatment of AOFH, especially for patients who are unwilling to undergo surgery.
