超声引导下微创旋切术对乳腺良性肿块患者围手术期指标及并发症的影响

摘　　要：目的:探讨超声引导下微创旋切术对乳腺良性肿块患者围手术期指标及并发症的影响。方法:选取2019年5月-2021年5月于龙南市第一人民医院治疗的60例乳腺良性肿块患者,按照随机数字表法将患者分为对照组(30例,予以传统开放式肿块切除术治疗)与研究组(30例,予以超声引导下微创旋切术治疗)。对比两组围手术期指标及康复指标;对比两组术后2、6、12及24 h时视觉模拟评分法(VAS)评分;对比两组术前、随访6个月时世界卫生组织生存质量测定量表(WHOQOL-100)评分;比较两组术后并发症发生率。结果:相较于对照组,研究组手术时间、切口愈合时间、住院时间、手术切口长度、瘢痕愈合长度均更短,术中出血量更少(P<0.05)。相较于对照组,研究组术后2、6、12、24 h时VAS评分均更低(P<0.05)。术后6个月,与术前相比,两组WHOQOL-100评分均升高,且研究组高于对照组(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:乳腺良性肿块患者应用超声引导下微创旋切术治疗,可减少术中出血量,缩小切口长度,缩短手术时间与术后康复进程,且能够减轻术后疼痛,提高生活质量。Objective: To investigate the effect of ultrasound-guided minimally invasive circumcision on postoperative indicators and complications in patients with benign breast masses. Method: A total of 60 patients with benign breast masses treated in the First People’s Hospital of Longnan City from May 2019 to May 2021 were selected, according to the random number table method, they were divided into control group(30 cases, treated with traditional open mass resection) and study group(30 cases, treated with ultrasound-guided minimally invasive circumcision). The postoperative indexes and rehabilitation indexes of the two groups were compared, and the visual analogue scale(VAS) scores of the two groups at 2, 6, 12 and 24 h after operation were compared;the scores of the World Health Organization quality of life(WHOQOL-100) before operation and at 6 months of follow-up were compared, and the incidence of postoperative complications was compared between the two groups. Result: Compared with the control group, the operation time, incision healing time, hospital stay, surgical incision length and healing scar length in the study group were shorter, and the intraoperative blood loss was less(P<0.05). Compared with the control group, the VAS scores in the study group were lower at 2, 6, 12 and 24 h after operation(P<0.05). 6months after operation, compared with the preoperative WHOQOL-100 score, the two groups were higher, and that in the study group was higher than that in the control group(P<0.05). There was no significant difference in the incidence of complications between the two groups(P>0.05). Conclusion: The application of ultrasound-guided minimally invasive circumcision in patients with benign breast masses can reduce the amount of bleeding during operation, shorten the length of incision, shorten the operation time and postoperative rehabilitation process, reduce postoperative pain and improve the quality of life.
