胃镜下高频电切联合氩等离子体凝固术对消化道息肉患者术后疼痛及胃肠功能的影响

摘　　要：目的:探讨胃镜下高频电切联合氩等离子体凝固术对消化道息肉患者术后疼痛及胃肠功能的影响。方法:选取2019年6月-2022年6月乐平市人民医院收治的消化道息肉患者72例,按照随机数字表法分为试验组(n=36,59枚息肉)、对照组(n=36,62枚息肉)。对照组给予胃镜下高频电切术治疗,试验组给予胃镜下高频电切联合氩等离子体凝固术治疗。比较两组手术指标(手术时间、术中出血量、住院时间、术后排气时间)、术后1 d疼痛程度[视觉模拟评分法(VAS)]、胃肠功能(I-FEED评分系统)、一次手术成功率及术后并发症发生情况。结果:试验组手术时间、术中出血量、住院时间及术后1 d VAS评分均优于对照组(P<0.05),两组术后排气时间比较,差异无统计学意义(P>0.05);两组I-FEED评分构成比比较,差异有统计学意义(P<0.05);两组一次手术成功率比较,差异无统计学意义(P>0.05);两组术后各并发症发生率比较,差异均无统计学意义(P>0.05)。结论:相较于胃镜下高频电切术,消化道息肉患者应用胃镜下高频电切联合氩等离子体凝固术可缩短手术时间及住院时间,减轻术后疼痛,在促进术后胃肠功能恢复方面更具优势。Objective:To explore the effects of gastroscope high-frequency electrotomy combined with argon plasma coagulation on postoperative pain and gastrointestinal function in patients with digestive tract polyp.Method:A total of 72 patients with digestive tract polyp who were admitted to Leping People’s Hospital from June 2019 to June 2022 were selected and divided into the experimental group(n=36,59 polyps) and the control group(n=36,62 polyps) according to the random number table.The control group was given gastroscope high-frequency electrotomy,and the experimental group was given gastroscope high-frequency electrotomy combined with argon plasma coagulation.The operation indexes(operation time,intraoperative bleeding volume,hospital stay and postoperative exhaust time),pain degree [visual analogue scale(VAS)] 1 d after operation,gastrointestinal function(I-FEED scoring system),success rate of one operation and incidence of postoperative complications were compared between the two groups.Result:The operation time,intraoperative bleeding volume,hospital stay and VAS scores of 1 d after operation in the experimental group were all better than those in the control group(P<0.05).There was no statistical significance in postoperative exhaust time between the two groups(P>0.05).The composition ratio of I-FEED score between the two groups was compared,the difference was statistically significant(P<0.05).There was no significant difference in the success rate of one operation between the two groups(P>0.05).There were no significant differences in the incidences of postoperative complications between the two groups(P>0.05).Conclusion:Compared with gastroscope high-frequency electrotomy,patients with digestive tract polyp applying gastroscope high-frequency electrotomy combined with argon plasma coagulation can shorten the operation time and hospital stay,reduce postoperative pain,and have more advantages in promoting the recovery of postoperative gastrointestinal function.
