摘  要:目的:观察频率特异性纯短音听性脑干反应(Tb-ABR)联合群组声刺激听性脑干反应(CsABR)在婴幼儿感音神经性耳聋听力诊断中的应用价值。方法:选取赣州市妇幼保健院2021年8月-2022年2月就诊的听力异常婴幼儿100例,所有患儿双耳均首先接受短声听性脑干反应(短声ABR)+CsABR检查、Tb-ABR+CsABR检查,分别记录检测结果,记为AC组、TC组,随后进行综合检查,作为“金标准”。对比两组对感音神经性耳聋诊断结果、诊断效能、不同程度感音神经性耳聋检出率。结果:经综合检测结果显示,100例(200耳)听力异常患儿中178耳为感音神经性耳聋;AC组检测结果显示,有166耳为感音神经性耳聋;TC组检测结果显示,有173耳为感音神经性耳聋;TC组敏感度96.07%、阴性预测值74.07%、准确率95.50%均高于AC组的89.89%、47.06%、88.00%,漏诊率3.93%低于AC组的10.11%,差异均有统计学意义(P<0.05);TC组对感音神经性耳聋轻度障碍的检出率高于AC组,差异有统计学意义(P<0.05)。结论:Tb-ABR+CsABR联合检查可提高轻度感音神经性耳聋患儿检出率,可为临床治疗方案的选择提供可靠依据。Objective:To observe the value of frequency specific tone burst auditory brainstem response(Tb-ABR) combined with chained-stimuli auditory brainstem response(CsABR) in the diagnosis of sensorineural hearing loss in infants.Method:A total of 100 infants with hearing impairment who were treated in Ganzhou Maternal and Child Health Hospital from August 2021 to February 2022 were selected.All the infants received short sound ABR + CsABR examination and Tb-ABR + CsABR examination.The test results were recorded respectively as AC group and TC group.The comprehensive examination was taken as the "gold standard".The diagnostic results,diagnostic efficacy and detection rates of sensorineural deafness in different degrees were compared between the two groups.Result:The results of comprehensive examination showed that 178 ears of 100 children(200 ears) with hearing abnormalities were sensorineural deafness;the results of AC group showed that 166 ears were sensorineural hearing loss;the results of TC group showed that 173 ears were sensorineural hearing loss;the sensitivity(96.07%),negative predictive value(74.07%) and accuracy(95.50%) in TC group were higher than 89.89%,47.06% and 88.00% in AC group,and the missed diagnosis rate(3.93%) was lower than 10.11% in AC group,with significant differences(P<0.05);the detection rate of mild sensorineural deafness disorder in TC group was higher than that in AC group,the difference was statistically significant(P<0.05).Conclusion:The combined examination of Tb-ABR and CsABR can improve the detection rate of children with mild sensorineural deafness and provide a reliable basis for the selection of clinical treatment.
