雌孕激素替代疗法联合阿普唑仑片治疗围绝经期综合征伴焦虑障碍的效果

摘　　要：目的:探讨雌孕激素替代疗法联合阿普唑仑片治疗围绝经期综合征伴焦虑障碍的临床效果。方法:选取2019年1月-2020年12月赣州市第三人民医院妇产科门诊或住院收治的70例围绝经期综合征伴焦虑障碍患者,通过随机数字表法将患者分为观察组和对照组,每组35例。对照组采用雌孕激素治疗,观察组采取雌孕激素疗法联合阿普唑仑片。比较两组治疗效果、汉密尔顿焦虑量表(HAMA)评分、血清激素[促卵泡生成素(FSH)、雌二醇(E_(2))、促黄体生成素(LH)]水平、围绝经期综合征生活质量表(MENQOL)评分、神经递质[5-羟色胺(5-HT)、β-内啡肽(β-EP)、去甲肾上腺素(NE)]水平。结果:与对照组(77.14%)相比,观察组治疗总有效率(94.29%)较高(P<0.05)。治疗后,观察组LH、FSH、HAMA评分和MENQOL评分均低于对照组,E_(2)高于对照组(P<0.05)。治疗后,观察组5-HT、β-EP水平均高于对照组,NE水平低于对照组,差异均有统计学意义(P<0.05)。结论:雌孕激素替代疗法联合阿普唑仑片治疗围绝经期综合征伴焦虑障碍患者对疗效有显著提升作用,维持激素水平平衡,减轻焦虑程度,改善睡眠质量与神经递质水平,且在提高患者生活质量方面有积极作用,值得临床推广应用。Objective:To explore the clinical efficacy of estrogen progesterone replacement therapy combined with Alprazolam Tablets in perimenopausal syndrome with anxiety disorder.Method:A total of 70 patients with perimenopausal syndrome and anxiety disorder admitted to the Department of Obstetrics and Gynecology of Ganzhou Third People’s Hospital from January 2019 to December 2020 were selected,they were divided into observation group and control group according to random table method,with 35 cases in each group.The control group received estrogen progesterone replacement therapy,and the observation group received estrogen progesterone replacement therapy combined with Alprazolam Tablets.The therapeutic effect,Hamilton anxiety scale(HAMA)scores,serum hormone[follicle stimulating hormone(FSH),estradiol(E_(2)),luteinizing hormone(LH)]levels,menopause specific quality of life questionnaire(MENQOL)scores,neurotransmitter[5-hydroxytryptamine(5-HT),β-endorphin(β-EP),noradrenaline(NE)]were compared between the two groups.Result:Compared with the control group(77.14%),the total effective rate of the observation group(94.29%)was higher(P<0.05).After treatment,the LH,FSH,HAMA score and MENQOL score of the observation group were lower than those of the control group,while E_(2) of the observation group was higher than that of the control group(P<0.05).After treatment,the levels of 5-HT andβ-EP of the observation group were higher than those of the control group,while the level of NE of the observation group was lower than that of the control group,the differences were statistically significant(P<0.05).Conclusion:Estrogen progesterone replacement therapy combined with Alprazolam Tablets in patients with perimenopausal syndrome with anxiety disorder can improve the efficacy,maintain the balance of hormone levels,reduce anxiety,improve sleep quality and neurotransmitter level,and have positive role in improving the quality of life of patients,which is worthy of clinical application.
