补肾活血法治疗强直性脊柱炎的Meta分析

摘　　要：目的:系统评价补肾活血法治疗强直性脊柱炎(AS)的效果。方法:检索中国知网、维普、万方、中国生物医学文献数据库、PubMed、Web of Science和Cochrane Library数据库中有关补肾活血法治疗AS的临床随机对照试验,检索时间为建库至2022年5月,采用RevMan 5.4对纳入文献进行Meta分析。结果:本研究纳入17篇文献,共1 230例患者。Meta分析结果表明,补肾活血法治疗AS在中医症候疗效[OR=4.09,95%CI(2.63,6.37),P<0.05]、红细胞沉降率[SMD=-0.66,95%CI(-1.03,-0.29),P<0.05]、C反应蛋白[SMD=-1.12,95%CI(-1.58,-0.67),P<0.05]、Bath强直性脊柱炎疾病活动度指数[SMD=-0.65,95%CI(-1.09,-0.22),P<0.05]、Bath强直性脊柱炎功能指数[SMD=-0.66,95%CI(-1.18,-0.15),P<0.05]、Schober试验[SMD=0.56,95%CI(0.35,0.77),P<0.05]、胸廓活动度[SMD=0.59,95%CI(0.20,0.99),P<0.05]、疼痛视觉模拟评分[MD=-0.90,95%CI(-1.34,-0.47),P<0.05]、晨僵时间[SMD=-0.69,95%CI(-0.91,-0.46),P<0.05]和不良反应[OR=0.45,95%CI(0.27,0.74),P<0.05]方面较对照组有明显优势。结论:补肾活血法治疗AS在提高疗效,降低炎性指标,改善疾病活动度及功能指数,保留及改善脊柱活动度,缓解疼痛和晨僵症状方面较对照组有明显优势,具有良好的安全性。Objective:To evaluate the clinical efficacy of Bushen Huoxue Recipe in the treatment of ankylosing spondylitis(AS) using Meta-analysis system.Method:A comprehensive collection of randomized controlled trials on treating AS with Bushen Huoxue Recipe was made by searching databases(from the establishment of bases to May 2022),such as CNKI,VIP,Wan Fang,PubMed,Web of Science and Cochrane Library.And finally the RevMan 5.4 software was used for Meta-analysis.Result:A total of 17 randomized controlled trials were included,with a total of 1 230 patients.Meta-analysis showed that the efficacy of Bushen Huoxue Recipe in the treatment of AS had significant advantages over the control group in TCM syndrome [OR=4.09,95%CI(2.63,6.37),P<0.05],ESR [SMD=-0.66,95%CI(-1.03,-0.29),P<0.05],CRP [SMD=-1.12,95%CI(-1.58,-0.67),P<0.05],BASDAI [SMD=-0.65,95%CI(-1.09,-0.22),P<0.05],BASFI [SMD=-0.66,95%CI(-1.18,-0.15),P<0.05],Schober test [SMD=0.56,95%CI(0.35,0.77),P<0.05],thoracic range of motion [SMD=0.59,95%CI(0.20,0.99),P<0.05],VAS scores [MD=-0.90,95%CI(-1.34,-0.47),P<0.05],morning stiffness time [SMD=-0.69,95%CI(-0.91,-0.46),P<0.05] and adverse reactions [OR=0.45,95%CI(0.27,0.74),P<0.05].Conclusion:The Bushen Huoxue Recipe in the treatment of AS has obvious advantages over the control group in improving the clinical effect,reducing the inflammatory index,and improving the disease activity and the functional index,preserving and improving the spinal mobility,relieving the symptoms of pain and morning stiffness,and has good security.
