摘　　要：目的:分析富血小板血浆(Prp)骨内浸润注射联合关节腔注射技术在胫骨平台骨折患者中的应用价值。方法:选取萍乡矿业集团有限责任公司总医院2019年5月-2021年11月SchafzkerⅡ~Ⅴ型胫骨平台骨折80例,并依照分型分层随机分成观察组和对照组,每组40例。对照组复位固定后采用Prp骨内浸润注射技术治疗,观察组在对照组的基础上加用Prp关节腔注射技术治疗。记录两组并发症发生情况;术后1、3、6个月对患者术后膝关节功能进行评估;术后6个月时采用Rasmussen评分评估患者骨折复位情况;术前及术后3个月检测患者血中C反应蛋白(CRP)、白细胞介素-1(IL-1)及肿瘤坏死因子-α(TNF-α)水平。结果:观察组术后并发症发生率低于对照组(P<0.05);术后3、6个月,观察组HSS评分均明显高于对照组(P<0.05);术后6个月,观察组Rasmussen评分均明显优于对照组(P<0.05);术后3个月,观察组血中CRP、IL-1及TNF-α水平均明显低于对照组(P<0.05)。结论:采用Prp骨内浸润注射联合关节腔注射技术对胫骨平台骨折患者治疗后可显著提升患者治疗质量并改善预后。Objective:To analyze the application value of platelet rich plasma(Prp)intraosseous infiltration injection combined with articular cavity injection technique in patients with tibial plateau fracture.Method:A total of 80 cases of SchafzkerⅡ-Ⅴtibial plateau fracture from May 2019 to November 2021 in the General Hospital of Pingxiang Mining Group Co.Ltd were selected and randomly divided into an observation group and a control group according to the classification and stratification,with 40 cases in each group.The control group was treated with intraosseous infiltration injection of Prp after reduction and fixation,the observation group was treated with Prp articular cavity injection technique on the basis of the control group.The complications of the two groups were recorded;1 month and 3,6 months after operation,postoperative knee joint function was evaluated;Rasmussen score was used to evaluate the fracture reduction at 6 months after operation;the levels of serum C reactive protein(CRP),interleukin-1(IL-1)and tumor necrosis factor-α(TNF-α)were detected before and 3 months after operation.Result:The incidence of postoperative complications in the observation group were significantly lower than those in the control group(P<0.05).3 and 6 months after operation,the HSS scores of the observation group were significantly higher than those of the control group(P<0.05).6 months after operation,the Rasmussen scores of the observation group were significantly better than those of the control group(P<0.05).3 months after operation,the levels of CRP,IL-1 and TNF-αin the observation group were significantly lower than those in the control group(P<0.05).Conclusion:Prp intraosseous infiltration injection combined with articular cavity injection technique in the treatment of patients with tibial plateau fractures can significantly improve the treatment quality and prognosis of patients with tibial plateau fracture after intervention.
