自拟生津方加减对糖尿病周围神经病变患者足部神经感觉的影响

摘　　要：目的:探讨自拟生津方加减治疗对糖尿病周围神经病变(DPN)患者足部神经感觉的影响。方法:选取2019年4月-2022年4月万载县中医院收治的DPN患者90例,根据随机数字表法将其分成生津方组与常规组各45例。常规组给予西药治疗,生津方组在常规组的基础上加用自拟生津方加减治疗。两组均治疗2个疗程后,评估中医疗效,比较两组治疗前、疗程结束时的中医证候积分。分别在两组治疗前、疗程结束时检测足跟、足背、前掌的皮肤温度及双足的震动感觉阈值(VPT),并比较两组不良反应发生率。结果:生津方组疗程结束时的主症、次症积分及总分均低于治疗前与常规组(P<0.05)。生津方组总有效率为93.33%,高于常规组的77.78%(P<0.05)。生津方组疗程结束时的足跟、足背、前掌温度高于治疗前与常规组(P<0.05)。生津方组疗程结束时双足VPT均低于治疗前与常规组(P<0.05)。生津方组(6.67%)不良反应发生率与常规组(2.22%)比较,差异无统计学意义(P>0.05)。结论:DPN患者采用自拟生津方加减治疗,能更有效提升中医疗效,改善患者的足部神经感觉功能,安全性高。Objective:To investigate the effect of self-made Shengjin prescription on foot nerve sensation in patients with diabetic peripheral neuropathy(DPN).Method:A total of 90 DPN patients admitted to Wanzai County Hospital of Traditional Chinese Medicine from April 2019 to April 2022 were selected,they were divided into Shengjin Prescription group and routine group with 45 cases in each group according to the random number table method.The routine group was given western medicine,and the Shengjin Prescription group was given the addition and subtraction therapy of self-made Shengjin Prescription on the basis of the routine group.After two courses of treatment,the curative effect of traditional Chinese medicine was evaluated,and the traditional Chinese medicine syndrome scores of the two groups before treatment and at the end of the course of treatment were compared.The skin temperature of heel,dorsum and forefoot and the vibrating perception threshold(VPT)of both feet were measured before treatment and at the end of treatment,and the incidence adverse reactions of the two groups were compared.Result:At the end of the course of treatment,the main and secondary syndrome scores and total scores of Shengjin Prescription group were lower than those before treatment and those of the routine group(P<0.05).The total effective rate of Shengjin Prescription group was 93.33%,which was higher than 77.78%of the conventional group(P<0.05).At the end of the course of treatment,the skin temperature of heel,dorsum and forefoot of the Shengjin Prescription group were higher than those before treatment and those of the routine group(P<0.05).At the end of the course of treatment,the VPT of both feet of Shengjin Prescription group were lower than those before treatment and those of the routine group(P<0.05).There was no difference in the incidence adverse reactions between the Shengjin Prescription group(6.67%)and the routine group(2.22%)(P>0.05).Conclusion:Self-made Shengjin Prescription can more effectively improve the curative effect of
