摘  要:目的:探讨妊娠期糖尿病(GDM)患者胎盘组织核因子E2相关因子2(Nrf2)/血红素加氧酶-1(HO-1)通路表达及其意义。方法:选取2019年1月-2021年12月潍坊市人民医院收治的280例GDM患者为GDM组,另选取同期正常足月妊娠孕妇100例作为对照组。两组分娩后均取胎盘组织,采用免疫组化法对胎盘组织Nrf2、HO-1进行检测。比较两组孕妇胎盘组织中Nrf2、HO-1表达水平并计算胰岛素抵抗指数(HOMA-IR)和胰岛素敏感指数(HOMA-IS);比较不同病理特征孕妇胎盘组织中Nrf2、HO-1表达水平;分析胎盘组织中Nrf2、HO-1表达水平与HOMA-IR、HOMA-IS的相关性。结果:GDM组孕妇胎盘组织中Nrf2、HO-1及HOMA-IR水平均高于对照组,HOMA-IS水平低于对照组(P<0.05)。不同空腹血糖、餐后1 h血糖、餐后2 h血糖、空腹胰岛素水平及新生儿体质量的GDM孕妇胎盘组织中Nrf2、HO-1表达比较,差异均有统计学意义(P<0.05)。经Pearson分析显示,Nrf2、HO-1表达与HOMA-IR水平均呈正相关,与HOMA-IS呈负相关(P<0.05)。结论:GDM患者胎盘组织中Nrf2、HO-1表达水平升高,且均与胰岛素抵抗相关,Nrf2/HO-1通路可能参与GDM发病过程。Objective:To investigate the expression of nuclear factor E_(2)-related factor(Nrf2)/heme oxygenase-1(HO-1)pathway in placental tissues of patients with gestational diabetes mellitus(GDM)and its significance.Method:A total of 280 GDM patients admitted to Weifang People’s Hospital from January 2019 to December 2021 were selected as the GDM group,and 100 normal full-term pregnant women were selected as the control group.Placental tissues of both groups were collected after delivery,and Nrf2 and HO-1 of placental tissues were detected by immunohistochemistry.The expression levels of Nrf2 and HO-1 in placental tissues of pregnant women were compared between the two groups,insulin resistance index(HOMA-IR)and insulin sensitivity index(HOMA-IS)were calculated;the expression levels of Nrf2 and HO-1 in placental tissues of pregnant women with different pathological features were compared;the correlation between the expression levels of Nrf2 and HO-1 in placental tissue and HOMA-IR,HOMA-IS were analyzed.Result:The levels of Nrf2,HO-1 and HOMA-IR in placental tissue of pregnant women in GDM group were higher than those in the control group,and the levels of HOMA-IS in GDM group was lower than that in the control group(P<0.05).There were significant differences in the expressions of Nrf2 and HO-1 in placental tissues of GDM pregnant women with different levels of fasting blood glucose,blood glucose 1 h after meal,blood glucose 2 h after meal,fasting insulin and neonatal body(P<0.05).Pearson analysis showed that the expression of Nrf2 and HO-1 expression were positively correlated with HOMA-IR level,and negatively correlated with HOMA-IS(P<0.05).Conclusion:The expression levels of Nrf2 and HO-1 in placental tissues of GDM patients are increased,and both of them are related to insulin resistance.Nrf2/HO-1 pathway may be involved in the occurrence and development of GDM.
