摘　　要：脑卒中后失语(post-stroke aphasia,PSA)作为一种高发且严重影响患者日常生活的脑卒中后遗症被越来越多的人关注,针刺治疗PSA凭借其安全、价格低廉、有效等优势,被纳入PSA的替代疗法,并得到国内外认可。针刺PSA的损伤机制及恢复机制仍不明确,基于临床量表的评估无法精准描述语言功能变化的脑机制,所以需要结合功能影像对PSA的脑活动进行描述。目前研究表明,针灸的作用机制可能与语言相关脑区的激活和功能连接有关,如左颞下回、边缘上回、中脑的Broca区和Wernicke区周围的脑区。本研究对功能核磁共振(functional magnetic resonance imaging,fMRI)在针刺治疗PSA的应用进行综述。提出并讨论目前存在争议的部分,对未来神经影像学用于PSA的针刺机制研究提供新思路。Post stroke aphasi(PSA),as a sequela of stroke with high incidence and serious impact on patients’daily life,has attracted more and more people’s attention.With its advantages of safety,low price and effectiveness,acupuncture treatment of PSA has been included in the alternative therapy of PSA,and has been recognized at home and abroad.The injury mechanism and recovery mechanism of acupuncture PSA are still unclear,and the evaluation based on clinical scales cannot accurately describe the brain mechanism of language function changes,so it is necessary to describe the brain activity of PSA in combination with functional images.Current research shows that the mechanism of acupuncture and moxibustion may be related to the activation and functional connection of language related brain regions,such as left inferior temporal gyrus,upper marginal gyrus,Broca region of midbrain and brain regions around Wernicke region.This study summarizes the application of functional magnetic resonance imaging(fMRI)in acupuncture treatment of PSA.It proposes and discusses the controversial parts at present,and provides a new idea for the future study of acupuncture mechanism of neuroimaging in PSA.
