主动健康视角下城市社区医养结合服务模式创新及其发展路径——以上海、青岛和杭州为例

摘　　要：随着我国人口老龄化加剧和疾病谱转变,树立主动健康观、将“治未病”理念融入社区医养结合服务体系成为政策的现实选择。基于主动健康概念内涵,可从服务主动性、功能活动的系统干预、服务资源整合性三个维度认识社区医养结合服务模式创新。从上海、青岛和杭州三个典型实践经验,可看出主动健康在社区医养结合服务模式中的具体体现及现有发展的不足。要从主动干预、提升服务可获得性、加强资源整合等方面,突出健康管理和个体健康责任的重要性,对老年人生理、心理和社会健康进行系统干预,以健康为中心实现资源整合和多部门协同,最大可能维持老年人的健康生活能力,以不断推进城市社区医养结合服务的高质量发展。With the aggravation of the aging population and the transformation of the disease spectrum in China,it has become a realistic policy choice to establish an active health concept and build a community medical and nursing service system with the core of"preventing disease".Based on the connotation of the concept of active health,the innovation of community medical care combined service mode is recognized from three aspects:service initiative,systematic intervention of functional activities and integration of service resources.Combined with the three typical practical experiences of Shanghai,Qingdao and Hangzhou,this paper expounds the concrete embodiment of active health in the community medical care service mode and the shortcomings of the existing development.From the aspects of active intervention,improving service availability and strengthening resource integration,it is proposed to highlight the importance of health management and individual health responsibility,carry out systematic intervention on the physical,psychological and social health of the elderly,realize resource integration and multi department coordination with health as the center,and maintain the healthy living ability of the elderly to the maximum extent,so as to continuously promote the innovation and high-quality development of the urban community integrated medical care service mode.
