复方荆芥熏洗剂熏洗联合RPH加外痔剥离切除术治疗重度混合痔38例疗效观察

摘　　要：目的:探讨复方荆芥熏洗剂熏洗联合痔疮自动套扎术(RPH)加外痔剥离切除术治疗重度混合痔的临床疗效。方法:将76例重度混合痔患者随机分为观察组和对照组,各38例。观察组采用复方荆芥熏洗剂熏洗联合RPH加外痔剥离切除术治疗,对照组采用外痔剥离切除术联合高锰酸钾坐浴治疗。比较2组疗效、康复时间、疼痛、水肿和肉芽组织生长情况等。结果:观察组总有效率为94.74%,高于对照组的73.68%(P <0.05);观察组渗出物消失时间、腐肉完全脱落时间、创面愈合时间和住院时间均短于对照组(P <0.05);术后4d和7d,观察组疼痛、水肿和肉芽组织生长评分均低于对照组(P <0.05);术后3周,观察组酸性和碱性成纤维细胞因子水平均高于对照组(P <0.05);观察组并发症发生率为5.26%,低于对照组的26.32%(P <0.05)。结论:复方荆芥熏洗剂熏洗联合RPH加外痔剥离切除术治疗重度混合痔,能够显著提高临床疗效,缩短患者术后康复时间,降低并发症发生率,提升血清生长因子水平。Objective To investigate the clinical effect of compound Jingjie wash-fumigant for fumingwash(short named:fuming-wash) combined with RPH & dissection resection of external hemorrhoids in the treatment of severe mixed hemorrhoids(SMH).Methods Randomly divided 76 SMH patients treated in author’s hospital into observation group(38 cases,fuming-wash+RPH+dissection resection of extenral hemorrhoids) and control group(38 cases,dissection resection of external hemorrhoids,and PPH solution for sitz bath);then, compared effect,rehabilitation time,as well as status on pain,edema,and granulation tissue growth, etc.between the two groups.Results In total effective rate;in the time for exudate disappeared,for slough completely lost,for wound-surface healing,and for hospitalization;the score ratings on pain,edema,and granulation,tissue growth at 4,and 7 days after surgery;the levels of basic fibroblast cytokine,and of acid ones;and,in the incidence of complication observation group was respectively higher(94.74% vs 73.68%);shorter;lower;higher;and lower(5.26% vs 26.32%) than control group(all,P <0.05).Conclusion Fuming-wash+RPH+dissection resection of external hemorrhoids for treating SMH can significantly enhance clinical effect,shorten postoperative rehabilitation time,reduce complication incidence,and uplift the levels of serum growth factors.
