骶管内注药治疗腰椎管狭窄症并发症危险因素分析

摘　　要：目的 探讨骶管内注药治疗腰椎管狭窄症(LSS)并发症的相关危险因素。方法 回顾性分析2017-01—2020-12贵州中医药大学第二附属医院麻醉科行骶管内注射治疗的638例LSS患者的临床资料。采用斯皮尔曼相关性分析各指标间的关系、利用倾向性评分匹配方法(PSM)对匹配后的数据分析并发症危险因素。结果 638例患者中43例发生并发症(6.73%)。经匹配后糖尿病与术后并发症的相关系数达0.53。高血压患者发生并发症的概率是无高血压患者的2倍,95%CI(0.87~3.07),糖尿病患者发生并发症的概率是非糖尿病患者的4.52倍,95%CI(3.44~5.77)。结论 骶管内注药治疗LSS,方法简单,并发症发生率低,疗效显著。高血压、糖尿病是骶管内注射并发症的危险因素。Objective To investigate the risk factors of complications of lumbar spinal stenosis(LSS) treated by sacral intraductal injection.Methods The clinical data of 638 patients with LSS who underwent sacral intraducral injection in the Department of Anesthesiology, the Second Affiliated Hospital of Guizhou University of Traditional Chinese Medicine from January 2017 to December 2020 were retrospectively analyzed.Spearman correlation was used to analyze the relationship between the indicators, and propensity score matching(PSM) was used to analyze the risk factors of complications.Results Complications occurred in 43 of 638 patients(6.73%).The correlation coefficient between diabetes and postoperative complications was 0.53.The incidence of complications in patients with hypertension was twice as high as that in patients without hypertension(95%CI: 0.87~3.07), and the incidence of complications in patients with diabetes was 4.52 times as high as that in patients without diabetes(95%CI: 3.44~5.77).Conclusion The treatment of LSS by sacral intraductal injection is simple, low complication rate and effective.Hypertension and diabetes mellitus are the risk factors for complications of intra sacral injection.
