摘　　要：目的评估Scarf联合Akin结合骨突植骨治疗中重度[母]外翻的短期疗效。方法回顾性分析2017年1月-2019年6月采用Scarf联合Akin结合骨突植骨治疗32例(45足)中重度[母]外翻患者的临床资料。结果术后切口均一期愈合,无切口相关并发症发生。全组患者均获随访,随访时间12~24个月,平均19.8个月。术后1足并发[母]内翻,2足出现轻度[母]僵硬,1足出现转移性跖痛,予以矫形鞋垫治疗后疼痛消失。截骨均愈合,愈合时间2~4个月,平均2.6个月。末次随访时HVA为13.1°±2.9°,1、2 IMA为4.9°±2.2°,均较术前显著改善,差异有统计学意义(P<0.05);MAA为20.9°±3.2°,与术前比较差异无统计学意义(P>0.05);AOFAS评分和VAS评分分别为(89.2±6.9)分、(1.5±0.9)分,均较术前显著改善,差异有统计学意义(P<0.05)。末次随访时满意度调查显示,患者非常满意及满意29例(90.6%),不满意3例(9.4%)。结论Scarf联合Akin结合骨突植骨治疗中重度[母]外翻短期疗效满意,值得临床推广。Objective To evaluate the short-term efficacy of scarf combined with akin combined with bone grafting in the treatment of moderate and severe hallux valgus.Methods The clinical data of 32 patients(45 feet)with moderate and severe hallux valgus treated with scarf combined with akin and bone grafting were analyzed retrospectively.Results All the incisions healed in the first stage,and no incision related complications occurred.All patients were followed up for 12~24 months,with an average of 19.8 months;One foot was complicated with valgus,two feet had mild stiffness,and one foot had metastatic plantar pain.The pain disappeared after orthopedic insole treatment.All osteotomies healed,and the healing time was 2~4 months,with an average of 2.6 months.At the last follow-up,HVA was(13.1±2.9)°and 1、2 IMA was(4.9±2.2)°,which were significantly improved compared with those before operation(P<0.05);The MAA was(20.9±3.2)°and there was no significant difference compared with that before operation(P<0.05).The scores of AOFAS and vas were(89.2±6.9)and(1.5±0.9)respectively,which were significantly improved compared with those before operation(P<0.05).The satisfaction survey at the last follow-up showed that 29 patients were very satisfied and satisfied(90.6%),and 3 patients were dissatisfied(9.4%).Conclusion Scarf combined with akin combined with bone process bone grafting has satisfactory short-term effect in the treatment of moderate and severe hallux valgus,which is worthy of clinical promotion.
