摘　　要：目的探讨应用瘢痕下蒂皮瓣修复手指瘢痕挛缩的临床疗效。方法2015年7月-2021年7月对40例手掌外伤后指间关节瘢痕挛缩畸形患者,采用瘢痕下蒂皮瓣推移修复指间关节瘢痕松解术后残余创面。术后观察皮瓣血运,指导患者康复训练,出院后随访观察患指关节活动及外观恢复情况。结果术后皮瓣无发黑、坏死,切口一期愈合,手指外形矫正。出院后随访3~12个月,平均(6.5±0.7)个月,皮瓣无回缩,与周围皮肤质地颜色相近,手部掌指关节功能活动明显改善。结论瘢痕下蒂皮瓣推移修复手部瘢痕挛缩畸形,无需植皮及二次手术修整,术后功能及外观改善明显,可为临床上解决手指瘢痕挛缩畸形提供新的方法。Objective To investigate the clinical effect of repairing finger scar contracture with subcicatricial pedicle flap.Methods A total of 40 patients with interphalangeal scar contracture deformity after palm trauma were admitted to our department from July 2015 to July 2021.The residual wounds after interphalangeal scar release were repaired with subcicatricial pedicle flap,blood supply of the flap was observed after surgery,rehabilitation exercise was guided,and activity and appearance recovery of the affected finger joint were observed after discharge.Results There was no black or necrosis of the skin flap after the operation,and the appearance of the finger was corrected after the incision had stage I healing.The follow-up period ranged from 3 months to 1 year after discharge with an average of 6.5±0.7 months.The skin flap did not retract,and the texture and color were similar to the surrounding skin,and the functional activity of the metacarpophalangeal joint of the hand was significantly improved.Conclusion The skin flap with subcicatricial pedicle can be used to repair the scar contracture deformity of hand without skin grafting and secondary surgical dressing,and the postoperative function and appearance can be improved significantly,which can provide a new method for the clinical treatment of finger scar contracture deformity.
