外固定支架在虎口挛缩治疗中的临床应用

摘　　要：目的探讨外固定支架治疗虎口挛缩的临床疗效。方法2019年1月-2021年1月,采用外固定支架治疗重度虎口挛缩30例,虎口角平均17.15°±4.27°,虎口宽度平均(2.44±0.51)cm。结果手术时间0.5~1 h,平均(0.5±0.5)h,出血量5.0~20.0 mL,平均(8.5±6.3)mL。患者均获得6个月随访。30例虎口挛缩得到缓解,拇指外展、对掌功能明显改善,外固定架治疗期间钉眼无一例出现红肿、渗出,虎口处拉伸皮肤均无异常感觉。按照Jensen及顾玉东等评价标准,虎口功能开大与手功能评价:优28例,良2例。结论外固定支架治疗虎口挛缩手术操作简单,效果良好。Objective To investigate the clinical efficacy of external fixation in the treatment of the first web space contracture.Methods Thirty patients with severe the first web space contracture were treated with external fixation.The average tiger mouth angle was(17.15±4.27)°.The average of the first web space width was(2.44±0.51)cm.Results Operation time was 0.5 to 1.0(0.5±0.5)h.Intraoperative bleeding was 5 to 20(8.5±6.3)mL.The patients were followed up for 6 months.30 patients got relief of the first web space contracture.Thumb abduction and palmar function were significantly improved.None of the patients showed redness,swelling,or oozing during the treatment of the external fixation nail hole,and none of them had abnormal sensation of the stretched skin at the the first web space.According to the evaluation criteria of Jensen and Gu Yudong,the functional opening of the the first web space and the evaluation of hand function were excellent in 28 cases and good in 2 cases.Conclusion The surgery of external fixation for treating the first web space contracture was simple and effective.
