摘  要:目的对比超微血流成像(SMI)与彩色多普勒超声(CDS)在肾动脉狭窄(RAS)诊断中的临床价值。方法回顾性分析2019年1月至2020年1月在本院收治的84例肾动脉狭窄患者的临床资料,所有患者均开展SMI(SMI组)与CDS(CDS组)检查,以数字减影血管造影(DSA)为诊断金标准,计算两组的临床诊断符合率、阴性预测值、阳性预测值、特异度、灵敏度、漏诊率、误诊率,并比较两组的诊断准确率。结果DSA检查显示168条肾动脉中,48条正常,18条轻度狭窄,24条中度狭窄,66条重度狭窄,12条闭塞。SMI组的诊断符合率为90.5%,阴性预测值为92.2%,阳性预测值为87.9%,漏诊率为12.1%,误诊率为7.8%,特异度为92.2%,灵敏度为87.9%;CDS组的诊断符合率为82.1%,阴性预测值为82.1%,阳性预测值为82.1%,漏诊率30.3%,误诊率为9.8%,特异度为90.2%,灵敏度为69.7%,两组的上述指标比较,差异均无统计学意义(均P>0.05)。SMI组的诊断准确率(81.0%)高于CDS组(66.7%),差异有统计学意义(P=0.003)。结论RAS诊断中SMI的应用效果显著,临床诊断准确率高于CDS,可用于筛查RAS。Objective To compare the clinical value of ultramicro blood flow imaging(SMI)and color Doppler ultrasound(CDS)in the diagnosis of renal artery stenosis(RAS).Methods The clinical data of 84 patients with renal artery stenosis admitted to our hospital from January 2019 to January 2020 were retrospectively analyzed.All patients underwent SMI(SMI group)and CDS(CDS group)examination,and digital subtraction angiography(DSA)was used as the gold standard for diagnosis.The clinical diagnostic coincidence rate,negative predictive value,positive predictive value,specificity,sensitivity,missed diagnosis rate and misdiagnosis rate of the two groups were calculated,and the diagnostic accuracy of the two groups was compared.Results DSA examination showed that of 168 renal arteries,48 were normal,18 were mildly stenotic,24 were moderately stenotic,66 were severely stenotic,and 12 were occluded.The diagnostic coincidence rate of the SMI group was 90.5%,the negative predictive value was 92.2%,the positive predictive value was 87.9%,the missed diagnosis rate was 12.1%,the misdiagnosis rate was 7.8%,the specificity was 92.2%,and the sensitivity was 87.9%;the diagnosis coincidence rate of the CDS group The negative predictive value was 82.1%,the negative predictive value was 82.1%,the positive predictive value was 82.1%,the missed diagnosis rate was 30.3%,the misdiagnosis rate was 9.8%,the specificity was 90.2%,and the sensitivity was 69.7%.There was no significant difference in the above indexes between the two groups(all P>0.05).The diagnostic accuracy rate of the SMI group(81.0%)was significantly higher than that of the CDS group(66.7%),and the difference was statistically significant(P<0.003).Conclusions The application effect of SMI in RAS diagnosis is significant,and the clinical diagnosis accuracy rate is higher than that of CDS,which can be used to screen RAS.
