自制比色卡法测定冲洗液颜色即刻计算泌尿外科手术中出血量的研究

摘　　要：目的寻找一种临床实用、简便快捷、术后即刻测定泌尿外科前列腺电切或经皮肾镜等手术术中出血量的方法,为手术及麻醉医生及时提供治疗参考。方法配置阶梯浓度冲洗液标本、照相、自制比色卡;选取本院2019年6月至2019年12月收治的250例前列腺等离子电切术及经皮肾镜碎石手术治疗患者,术中收集所有冲洗液,术毕即刻用比色卡法计算术中出血量,并收集冲洗液标本,再用肉眼估算法和血常规法测定出血量,对比分析比色卡法结果的准确性。结果比色卡法、肉眼估算法、血常规法的预估出血量分别为(76.68±63.65)、(92.73±62.27)、(77.20±63.55)mL,比色卡法预估出血量与肉眼估算法预估出血量比较,差异有统计学意义(t=2.850,P=0.005);比色卡法预估出血量与血常规法预估出血量比较,差异无统计学意义(t=0.091,P=0.927);肉眼估算法预估出血量与血常规法预估出血量比较,差异有统计学意义(t=2.760,P=0.006)。结论与肉眼估算法和血常规法预估出血量相比,自制比色卡法测定含冲洗液标本的术中出血量具有成本低、方法简单、即刻准确、简便快捷的优点,适合各类外科手术的出血量分析,具有广阔的应用前景,值得各级医院推广。Objective To find a clinically practical,simple and quick method to measure intraoperative blood loss in urological resection of prostate or percutaneous nephroscopy immediately after operation,so as to provide reference for operation and anesthesiologist in time.MethodssEquipped with step concentration rinsing liquid specimen,photography,self-made standard"colorimetric card";from June 2019 to December 2019,250 patients who underwent plasma resection of prostate and percutaneous nephrolithotomy in our hospital were selected.All the flushing fluid was collected during the operation,and the intraoperative blood loss was calculated by the"colorimetric card"method immediately after the operation,and the specimen of flushing fluid was collected,and then the blood loss was measured by the macroscopic estimation method and the blood routine method,and the accuracy of the results of the colorimetric card method was compared and analyzed.Results The estimated blood loss of color card method,naked eye estimation method and blood routine method were(76.68±63.65)mL,(92.73±62.27)mL and(77.20±63.55)mL,respectively.There was statistically significant difference in the estimated blood loss between"colorimetric card"method and macroscopic estimation method(t=2.85,P=0.005).There was no significant difference in the amount of blood loss predicted by colorimetric card method and blood routine method(t=0.091,P=0.927).There was statistically significant difference in the estimated blood loss between the macroscopic estimation method and the blood routine method(t=2.76,P=0.006).Conclusions Compared with the naked eye estimation method and the blood routine method,the self-made colorimetric card method has the advantages of low cost,simple method,immediate accuracy,simple and quick determination of the blood loss of the specimen containing rinsing liquid.It is suitable for the blood loss analysis of all kinds of surgical operations.It has broad application prospect and is worth popularizing in hospitals at all levels.
