摘 要:目的探查与输尿管结石粘连的息肉组织中是否存在结石相关功能蛋白的表达,探讨了蛋白在结石息肉粘连、结石生长中的作用。方法选取2020年1月至2020年6月在山东省结石病防治中心收治的20例输尿管结石合并息肉患者,共收集40例息肉标本。根据息肉组织是否与结石粘连分为观察组和对照组,每组各20例。观察组为与结石粘连的息肉组织,对照组为没有与结石粘连的息肉组织。对息肉组织进行病理学检查,明确息肉组织中结石相关功能蛋白(膜联蛋白A1、草酸钙结合蛋白S100A9、骨桥蛋白和尿调素蛋白)的表达量。结果两组均存在急慢性输尿管炎和局部尿路上皮增生。与对照组比较,观察组的炎症细胞明显增多。免疫组化染色结果显示膜联蛋白A1和草酸钙结合蛋白S100A9均表达于尿路上皮细胞,但是与对照组比较,观察组的蛋白染色强度显著增加。尿调素蛋白和骨桥蛋白在两组标本中均未显著表达。与对照组比较,观察组的膜联蛋白A1和草酸钙结合蛋白S100A9的蛋白表达量增加(均P<0.05),而观察组和对照组的尿调素蛋白和骨桥蛋白表达均无明显差异(均P>0.05)。Pearson相关性分析结果显示,膜联蛋白A1与草酸钙结合蛋白S100A9表达量存在正相关(r=0.741,P=0.022)。结论与输尿管结石粘连的息肉组织中存在高表达的膜联蛋白A1和草酸钙结合蛋白S100A9,这两种蛋白既是炎症信号通路中的关键蛋白,也是尿石症中蛋白信号通路的上、下游蛋白,这两种蛋白可能在结石与息肉粘连、结石生长中扮演关键角色。Objective To explore the expressions of calculus-related functional proteins in the ureteral calculus-adhered polyp tissues and investigate the role of these proteins in the formation of adhesions between the calculus and polyp.Methods A total of 20 patients with ureteral calculi complicated with polyps admitted to Shandong stone disease prevention and treatment center from January 2020 to June 2020 were selected,and a total of 40 polyp specimens were collected.According to whether the polyp tissue is adhered to the stone,the patients were divided into an observation group and a control group,with 20 cases in each group.The observation group was the polyp tissue adhered to the stone,and the control group was the polyp tissue that was not adhered to the stone.Pathological examination of polyp tissue was performed to identify stone-related functional proteins(expressions of annexin A1,calcium oxalate-binding protein S100A9,osteopontin and uromodulin)in polyp tissue.Results There were acute and chronic ureteritis and local urothelial hyperplasia in both groups.Compared with the control group,the inflammatory cells in the observation group were significantly increased.The results of immunohistochemical staining showed that both annexin Al and calcium oxalate-binding protein S100A9 were expressed in urothelial cells,but compared with the control group,the protein staining intensity of the observation group was significantly increased.Uromodulin protein and osteopontin were not significantly expressed in the two groups of specimens.Compared with the control group,the protein expressions of annexin Al and calcium oxalate-binding protein S100A9 in the observation group were increased(all P<0.05),while the expressions of uromodulin and osteopontin were not significantly different between the observation group and the control group(all P>0.05).The results of Pearson correlation analysis showed that there was a positive correlation between the expression of annexin Al and calcium oxalate-binding protein S100A9(r=0.741,P=0.0
