输尿管镜联合简易负压装置治疗输尿管上段结石的有效性与安全性

摘　　要：目的探讨简易负压装置联合输尿管半硬镜治疗输尿管上段结石的有效性及安全性。方法回顾性分析2016年1月至2017年7月本院收治的58例输尿管上段结石患者的临床资料,按照不同的治疗方式分为试验组(27例)和对照组(31例),试验组行输尿管半硬镜联合简易负压装置碎石,对照组行单纯输尿管半硬镜碎石。比较两组患者的年龄、结石直径、术后住院时间、手术时间、结石清除率及并发症。结果试验组的结石直径为(10.56±2.22)mm,对照组为(10.52±1.84)mm,两组差异无统计学意义(P>0.05)。试验组的术后住院时间为(2.78±1.34)d,对照组为(2.97±0.65)d,两组差异无统计学意义(P>0.05)。试验组及对照组的手术时间分别为30 min、51 min。试验组和对照组的结石清除率分别为96.30%、71.97%。两组手术时间及术后结石清除率比较,差异均有统计学意义(均P<0.05)。两组手术患者均无明显并发症发生。结论输尿管半硬镜结合简易负压装置治疗输尿管上段结石,操作简单,手术时间短,结石清除率高,是一种值得推广的方法。Objective To assess the safety and efficacy of negative pressure suctioning combined with ureteroscope for treating upper ureteral stones.Methods The clinical data of 58 patients with upper ureteral calculi admitted to our hospital from January 2016 to July 2017 were retrospectively analyzed.According to different treatment methods,they were divided into experimental group(27 cases)and control group(31 cases).The experimental group was treated with semi-rigid ureteroscope combined with simple negative pressure device,while the control group was treated with simple semirigid ureteroscope lithotripsy.The age,stone diameter,postoperative hospital stay,operation time,stone clearance rate and complications were compared between the two groups.Results The stone diameter of the experimental group was(10.56±2.22)mm,and that of the control group was(10.52±1.84)mm,and there was no significant difference between the two groups(P>0.05).The postoperative hospital stay in the experimental group was(2.78±1.34)d,and that in the control group was(2.97±0.65)d,and there was no significant difference between the two groups(P>0.05).The operation time of experimental group and control group was 30 min and 51 min,respectively.The stone clearance rates of experimental group and control group were 96.30%and 71.97%,respectively.There were significant differences in operation time and postoperative stone clearance rate between the two groups(all P<0.05).There were no significant complications in the two groups.Conclusions Semi-rigid ureteroscope combined with negative pressure suctioning has the advantages of simple operation,short operation time and high stone clearance rate.It is a method worthy of popularization in the treatment of upper ureteral calculi.
