探讨先天性色素痣合并增生性结节的临床特点及组织病理特征

摘　　要：目的 探讨先天性色素痣(CMN)合并增生性结节(PN)的临床特点及组织病理特征。方法 回顾性分析2016年1月—2020年12月本市中心医院7例CMN合并PN患者资料,收集患者临床资料及病理特征资料,分析CMN合并PN的临床特点及组织病理特征。结果 7例CMN合并PN病例中男4例(57.14%)、女3例(42.86%);7例患者中成人仅1例(14.29%),其余6例均为年龄小于10岁的儿童;从痣大小来看,7例患者中小型CMN 2例(28.57%)、中型CMN 3例(42.86%)、大型CMN 2例(28.57%);从痣类型来看,7例患者中混合痣6例(85.71%),皮内痣1例(14.29%),病理检查PN细胞内可见核分裂象2例(28.57%),所有CMN合并PN病例均可见成熟现象,2例(28.58%)患者可见Paget样分布。结论 PN多发生于中大型CMN患者婴幼儿时期,PN内细胞无明显的异型性但可见明显的成熟现象,部分PN细胞内可见核分裂象与Paget样分布。Objective To investigate the clinical and histopathological characteristics of congenital pigmented nevi(CMN)complicated with hyperplastic nodules(PN).Methods The data of 7 patients suffered with CMN and PN those were admitted to Heze central hospitalfrom January 2016 to December 2020 were analyzed retrospectively.The clinical and pathological data of patients were collected, and the clinical and histopathological characteristics of CMN combined with PN were analyzed.Results Of the 7 cases of CMN complicated with PN,4 cases were male(57.14%)and 3 cases were female(42.86%).Of the 7 cases, only 1 case was adult(14.29%)and the other 6 cases were children younger than 10 years old.Of the 7 cases of CMN with PN,2 cases were small CMN(28.57%),3 cases were medium CMN(42.86%)and 2 cases were large CMN(28.57%).Of the 7 cases, 6 cases were mixed nevus(85.71%)and 1 case was intradermal nevus(14.29%).Intracellular nuclear division was seen in 2 cases in pathological examination(28.57%).Maturation was seen in all cases of CMN with PN,and 2 cases(28.58%)with Paget pattern.Conclusions PN mostly occurs in patients with medium to large CMN during their early childhood.Mitotic image and Paget distribution can be seen in some PN cells.
