耳内镜下耳屏软骨-软骨膜和耳屏软骨膜修补鼓膜大穿孔疗效分析

摘　　要：目的探讨耳内镜下耳屏软骨-软骨膜和耳屏软骨膜修补鼓膜大穿孔的疗效。方法回顾2020年1月~2021年12月在扬州大学临床医学院接受鼓膜修补手术的97例(97耳)患者,随机分成两组,分别为采用耳内镜下耳屏软骨-软骨膜修补鼓膜穿孔48例和采用耳内镜下耳屏软骨膜修补鼓膜穿孔49例,术后1、3、6、12个月复查,至少随访3个月以上。比较两组术后3个月以上鼓膜愈合率及听力情况。结果术后3个月软骨-软骨膜组鼓膜愈合率95.83%(46/48),软骨膜组鼓膜愈合率95.92%(47/49)(χ^(2)=0.000,P>0.05);术后3个月平均气导阈值均较术前提高,两组差异无统计学意义(t=-0.404,P>0.05)。结论耳内镜下软骨-软骨膜修补鼓膜穿孔与软骨膜修补疗效相当,是一种有效的手术方法,值得临床推广。OBJECTIVE To investigate the effect of endoscopic repair of tympanic membrane perforation with cartilage perichondrium and perichondrium.METHODS 97 cases(97 ears)who underwent tympanic membrane repair in our hospital from January 2020 to December 2021 were reviewed.They were randomly divided into two groups.48 cases in the first group were repaired with tragus cartilage perichondrium under otoendoscope,and 49 cases in another group were repaired with tragus perichondrium under otoendoscope.They were rechecked at 1,3,6 and 12 months after operation,and followed up for at least 3 months.The healing rate of tympanic membrane and hearing were compared between the two groups at more than 3 months after operation.RESULTS Three months after operation,the tympanic membrane healing rate of cartilage perichondrium group was 95.83%,and that of perichondrium group was 95.92%(χ^(2)=0.000,P>0.05).The average air conduction threshold at three months after operation was higher than that before operation(t=-0.404,P>0.05).There was no significant difference between the two groups.CONCLUSION The effect of endoscopic repair of tympanic membrane perforation using cartilage perichondrium is equivalent to that of perichondrium.It is an effective surgical method and worthy of clinical recommendation.
