经阴道彩色多普勒超声对未破裂型异位妊娠的鉴别诊断价值

摘　　要：目的 观察经阴道彩色多普勒超声(彩超)对未破裂型异位妊娠(EP)与妊娠黄体囊肿的鉴别诊断价值,指导临床实施合理治疗。方法 回顾性分析本院2018年1月至2020年1月收治52例未破裂型EP患者及同期收治的43例妊娠黄体囊肿患者资料,以腹腔镜检查结果为金标准,均行经阴道彩超检查,记录未破裂型EP与妊娠黄体囊肿的包块形态及血流特征,评估经阴道彩超检查对未破裂型EP与妊娠黄体囊肿的鉴别诊断价值。结果 未破裂型EP患者与妊娠黄体囊肿患者环壁厚度、包块直径比较,差异无统计学意义(P>0.05);未破裂型EP患者阻力指数及搏动指数均高于妊娠黄体囊肿患者,差异有统计学意义(P<0.05);未破裂型EP患者与妊娠黄体囊肿患者血流分布比较,差异有统计学意义(P<0.05);经阴道彩超与腹腔镜检查结果的一致性好;经阴道彩超的诊断灵敏度、特异度及准确度均高于90.00%。结论 经阴道彩超对未破裂型EP与妊娠黄体囊肿的鉴别诊断价值较高,可通过包块血流特征,有效鉴别两者,诊断灵敏度、特异度及准确度较为理想。Objective To observe the value of transvaginal color Doppler ultrasound(TVCDU)in the differential diagnosis of unruptured ectopic pregnancy(EP)and pregnancy corpus luteum cyst,and to guide the clinical treatment of patients.Methods The data of 52 patients with unruptured EP and 43 patients with gestational luteal cyst treated in our hospital from January 2018 to January 2020 were retrospectively analyzed.Taking the results of laparoscopy as the gold standard,transvaginal color Doppler ultrasonography was performed to record the mass morphology and blood flow characteristics of unruptured EP and gestational luteal cyst,To evaluate the value of transvaginal color Doppler ultrasonography in the differential diagnosis of unruptured EP and pregnancy corpus luteum cyst.Results There was no significant difference in annular wall thickness and mass diameter between unruptured EP patients and pregnant luteal cyst patients(P>0.05);The resistance index and pulsatility index in patients with unruptured EP were higher than those in patients with gestational luteal cyst(P<0.05);There was significant difference in blood flow distribution between unruptured EP patients and pregnant luteal cyst patients(P<0.05);The consistency between transvaginal color Doppler ultrasound and laparoscopy was good;The sensitivity,specificity and accuracy of transvaginal color Doppler ultrasound were higher than 90.00%.Conclusion Transvaginal color Doppler ultrasound has high value in the differential diagnosis of unruptured EP and gestational corpus luteum cyst.It can effectively distinguish them through the blood flow characteristics of mass.The diagnostic sensitivity,specificity and accuracy are ideal.
