摘 要:目的采用微电极阵列标测技术测定,评价七氟烷预处理心脏成纤维细胞来源外泌体对低温缺血再灌注大鼠离体心脏心室肌电传导的影响。方法SPF级SD大鼠乳鼠,雌雄不拘,差速贴壁法提取原代心脏成纤维细胞。2~4代心脏成纤维细胞用2.5%七氟烷持续处理1 h,继续培养24~48 h后提取外泌体。SPF级健康雄性SD大鼠,2~3月龄,体重280~320 g,按照随机数字表法分为3组(n=8):对照组(C组)、缺血再灌注组(IR组)和七氟烷预处理心脏成纤维细胞源性外泌体+缺血再灌注组(S+IR组)。C组平衡灌注110 min;IR组和S+IR组平衡灌注20 min,停灌60 min后再灌注30 min。S+IR组于术前48 h尾静脉注射七氟烷预处理心脏成纤维细胞分泌的外泌体1 ml(200μg),C组和IR组分别注射生理盐水1 ml。于平衡灌注20 min(T_(0))、再灌注15 min(T_(1))和再灌注30 min(T_(2))时采用微电极阵列在离体心脏左心室表面,获取心脏传导速度(CV)、传导绝对不均一性(P_(5-95))和不均一性指数(P_(5-95)/P_(50))。结果与C组比较,S+IR组T_(1)时CV降低,P_(5-95)和P_(5-95)/P_(50)增加(P<0.05),T_(2)时差异无统计学意义(P>0.05),IR组T_(1,2)时CV减慢,P_(5-95)和P_(5-95)/P_(50)升高(P<0.05);与IR组比较,S+IR组T_(1,2)时CV增加,P_(5-95)和P_(5-95)/P_(50)降低(P<0.05)。结论七氟烷预处理心脏成纤维细胞源性外泌体可改善低温缺血再灌注大鼠离体心脏心室肌的电传导。Objective To evaluate the effects of exosomes derived from cardiac fibroblasts pretreated with sevoflurane on ventricular electrical conduction in isolated rat hearts subjected to hypothermic ischemia-reperfusion(I/R)using the multi-electrode array mapping technique.Methods Primary cardiac fibroblasts were extracted by differential adhesion in SPF Sprague-Dawley rats of either sex.Cardiac fibroblasts of passage 2-4 were treated with 2.5%sevoflurane for 1 h,and then cultured for 24-48 h to extract exosome.SPF healthy male Sprague-Dawley rats,aged 2-3 months,weighing 280-320 g,were divided into 3 groups(n=8 each)using a random number table method:control group(group C),I/R group and sevoflurane-pretreated cardiac fibroblast-derived exosome+IR group(group S+IR).Hearts were perfused for 110-min equilibration in group C.After 20 min of equilibration,the perfusion was suspended for 60 min(global ischemia)followed by 30 min of reperfusion in IR and S+IR groups.Exosomes 1 ml(200μg)derived from cardiac fibroblasts pretreated with sevoflurane were injected through the tail vein at 48 h before surgery in group S+IR,and the equal volume of normal saline was injected instead in C and IR groups.The cardiac conduction velocity(CV),conduction absolute inhomogeneity(P_(5-95))and inhomogeneity index(P_(5-95)/P_(50))were obtained at 20 min of equilibration(T_(0))and 15 and 30 min of reperfusion(T_(1,2))using the microelectrode array attaching to the left ventricular surface of the isolated heart.Results Compared with group C,CV was significantly decreased and P_(5-95) and P_(5-95)/P_(50) were increased at T_(1)(P<0.05),and no significant change was found at T_(2) in group S+IR(P>0.05),and CV was significantly decreased and P_(5-95) and P_(5-95)/P_(50) were increased at T_(1,2) in group IR(P<0.05).Compared with group IR,CV was significantly increased and P_(5-95) and P_(5-95)/P_(50) were decreased at T_(1,2) in group S+IR(P<0.05).Conclusions Exosomes derived from cardiac fibroblasts pretreated with sevoflurane can improve ventricul
