24小时动态血压监测仪在中老年高血压合并冠心病患者中的应用评价

摘　　要：目的探究在中老年高血压合并冠心病患者血压监测中应用24小时动态血压监测仪的价值。方法选择2019年5月-2020年12月在本院治疗的99例高血压合并冠心病患者组成观察组,另选同期住院治疗的单纯高血压患者90例组成对照组,患者全部使用24小时动态血压监测仪监测血压,对比两组患者日间和夜晚血压值以及24小时血压平均值,同时对比两组患者发生白大衣高血压的概率。结果观察组24小时舒张压和收缩压平均值相比于对照组更高,数据对比差异显著,存在统计学意义(P<0.05)。观察组患者日间与夜晚的舒张压水平、收缩压水平与对照组相比均更高,数据对比差异存在统计学意义(P<0.05)。观察组患者舒张压、收缩压水平夜间与日间的差值明显大于对照组,差异有统计学意义(P<0.05)。观察组24小时舒张压总负荷显著大于对照组,24小时收缩压总负荷大于对照组,差异有统计学意义(P<0.05)。观察组患者发生白大衣高血压的概率明显低于对照组,差异有统计学意义(P<0.05)。结论在中老年高血压合并冠心病患者的血压监测中使用24小时动态血压监测仪可以获得良好应用效果,患者24小时内的舒张压和收缩压变化、波动情况都可以准确记录下来,为药物治疗效果评价、病情进展预测提供了可靠依据,值得应用。Objective To explore value of 24-hour ambulatory blood pressure monitoring in middle-aged and elderly patients with hypertension and coronary heart disease.Methods The paper chose 99 patients with hypertension and coronary heart disease treated in our hospital from May 2019 to December 2020 as observation group,and 90 patients with simple hypertension hospitalized in the same period as control group.All patients were treated with 24-hour ambulatory blood pressure monitor to monitor their blood pressure,daytime and nighttime blood pressure levels and 24-hour mean blood pressure between two groups were compared.At the same time,probability of white coat hypertension of two groups was compared.Results Mean values of 24-hour diastolic and systolic blood pressure in observation group were higher than control group,with significant difference in data comparison,(P<0.05).Diastolic blood pressure and systolic blood pressure of observation group during day and night were higher than control group,with statistically significant difference in data comparison,(P<0.05).Difference of diastolic blood pressure and systolic blood pressure between night and day in observation group was significantly bigger than control group(P<0.05).Total load of 24-hour diastolic blood pressure in observation group was significantly higher than control group,total load of 24-hour systolic blood pressure was significantly higher than control group(P<0.05).Probability of white coat hypertension in observation group was significantly lower than control group(P<0.05).Conclusion 24-hour ambulatory blood pressure monitor can obtain good application effect in blood pressure monitoring of middle-aged and elderly patients with hypertension and coronary heart disease,can accurately recorde changes and fluctuations of diastolic and systolic blood pressure of patients within 24-hour,provide reliable basis for evaluation of drug treatment effect and prediction of disease progress,which is worthy of application.
