摘 要:目的探讨无创呼吸机治疗慢性阻塞性肺疾病合并呼吸衰竭患者的效果。方法选取我院慢性阻塞性肺疾病合并呼吸衰竭患者,随机分为常规组、呼吸机组,采用对照试验方法探讨分别应用常规治疗、无创呼吸机支持治疗的治疗效果。结果呼吸机组显效32例,占比64.00%,有效16例,占比32.00%,无效2例,占比4.00%,总有效率96.00%;常规组显效22例,占比44.00%,有效15例,占比30.00%,无效13,占比26.00%,总有效率74.00%。呼吸机组的总有效率高于常规组(χ^(2)=10.25,P<0.05);呼吸机组的肺通气功能指标显著优于常规组,P<0.05;治疗后,常规组的PaO_(2)为(72.70±4.80)mm Hg,Pa CO_(2)为(71.90±5.40)mm Hg,呼吸机组Pa O2为(85.10±5.70)mm Hg,Pa CO_(2)为(54.80±5.10)mm Hg,呼吸机组的PaO_(2)、Pa CO_(2)优于常规组(t=5.69、6.29,P<0.05)。结论慢性阻塞性肺疾病合并呼吸衰竭患者采用无创呼吸机支持治疗的效果显著,有助于改善肺通气功能。Objective To explore the clinical effect of noninvasive ventilator in the treatment of chronic obstructive pulmonary disease with respiratory failure.Methods Patients with chronic obstructive pulmonary disease complicated with respiratory failure in our hospital were randomly divided into general group and ventilator group,and the treatment effect of conventional treatment and non-invasive ventilator supportive therapy was explored by controlled trial method.Results In the ventilator group,32 cases were markedly,accounting for 64.00%,16 cases were effective,accounting for 32.00%,2 cases were ineffective,accounting for 4.00%,and the total effective rate was 96.00%;in the general group,22 cases were markedly,accounting for 44.00%,15 cases were effective,accounting for 30.00%,13 cases were ineffective,accounting for 26.00%,and the total effective rate was 74.00%.The total effective rate of the ventilator group was higher than that of the general group,and the difference between the two groups was significant(χ^(2)=10.25,P<0.05);the pulmonary ventilation index of the ventilator group was significantly better than that of the general group,P<0.05;after treatment,PaO_(2) of the general group was(72.70±4.80)mm Hg,PaCO2 was(71.90±5.40)mm Hg,PaO_(2) of the ventilator group was(85.10±5.70)mm Hg,and PaO_(2) was(54.80±5.10)mm Hg.The PaO_(2) and PaO_(2) were significantly different between the two groups(t=5.69,6.29,P<0.05).Conclusion The effect of non-invasive ventilator support in COPD with respiratory failure is significant,and it can improve the pulmonary ventilation function.
