四种方法拔除下颌低位阻生智齿的效果观察

摘　　要：目的:比较传统骨凿去骨劈冠法、高速涡轮钻法、超声骨刀法、超声骨刀联合高速涡轮钻法四种方法拔除下颌低位阻生智齿的效果及安全性。方法:收集2017年6月—2019年6月在天津市宝坻区人民医院行下颌低位阻生智齿拔除的123例患者作为研究对象,根据其拔牙方法分为骨凿组(n=30)、高速涡轮组(n=30)、超声骨刀组(n=31)、超声骨刀联合高速涡轮组(n=32)。比较四组患者手术时间、术后张口受限程度、手术并发症发生情况。结果:超声骨刀组手术时间与高速涡轮组、超声骨刀联合高速涡轮组之间差异有统计学意义(P<0.05);高速涡轮组手术时间与超声骨刀联合高速涡轮组相比,差异无统计学意义(P>0.05);超声骨刀组术后张口受限评分与高速涡轮组、超声骨刀联合高速涡轮组相比,差异有统计学意义(P<0.05);高速涡轮组术后张口受限评分与超声骨刀联合高速涡轮组相比,差异无统计学意义(P>0.05);高速涡轮组手术总并发症发生率与超声骨刀组、超声骨刀联合高速涡轮组之间比较,差异无统计学意义(P>0.05)。结论:超声骨刀联合高速涡轮拔除下颌低位阻生智齿可结合两种方法的优点,能够有效缩短拔牙时间,减轻张口受限,且安全性较高。Objective:To compare the effectiveness and safety of extraction of mandibular low impacted wisdom teeth by conventional bone gouge debridement and crown splitting method, high speed turbo drill method, ultrasonic bone knife method and ultrasonic bone knife combined with high speed turbo drill method.Methods:123 patients with 123 mandibular low obstructive wisdom teeth who underwent extraction of mandibular low obstructive wisdom teeth in the hospital from June 2017 to June 2019 were collected and divided into 30 cases in the bone chisel group, 30 cases in the high-speed turbine group, 31 cases in the ultrasonic bone knife group, and 32 cases in the ultrasonic bone knife combined with high-speed turbine group according to the extraction method. The duration of surgery, the degree of postoperative mouth opening restriction, and the occurrence of surgical complications were compared among the four groups of patients.Results:The difference in operative time between the ultrasonic bone knife group and the high-speed turbine group and the ultrasonic bone knife combined with high-speed turbine group was statistically significant(P<0.05). There was no statistically significant difference in operative time in the high-speed turbine group compared with the ultrasonic bone knife combined with high-speed turbine group(P>0.05). There was statistically significant difference in the postoperative opening restriction score in the ultrasonic bone knife group compared with the high-speed turbine group and the ultrasonic bone knife combined with high-speed turbine group(P<0.05). There was no statistically significant difference in the postoperative mouth opening restriction score in the high-speed turbine group compared with the ultrasonic bone knife combined with high-speed turbine group(P>0.05). There was no statistically significant difference between the total surgical complication rate in the high-speed turbine group and the ultrasonic bone knife group and the ultrasonic bone knife combined with high-speed turbine group(P>0.05
