摘 要:目的 探讨颈围与24 h动态血压参数的关联。方法 选取在2014-2019年期间完成随访且体检资料完整,并接受24 h动态血压监测(24hABPM)的开滦研究人群作为观察对象。根据不同性别颈围的三分位数将观察对象分为三组,采用广义线性模型分析不同分位组观察对象颈围与24hABPM参数之间的关联,并根据观察对象是否患高血压进行分层分析。结果 符合纳入标准的观察对象共2 504例,其中男性1 709例(68.2%),观察对象平均年龄为(63.0±10.8)岁,平均颈围为(38.1±3.6)cm。校正年龄、性别、吸烟、饮酒、受教育程度、总胆固醇、高敏C反应蛋白、服用降压药、打鼾、糖尿病史、体育锻炼情况和体质量指数(BMI)后,广义线性模型分析结果示:以低颈围组为对照,高颈围组24 h平均收缩压(24hSBP)、24 h平均舒张压(24hDBP)、日间平均收缩压(dSBP)、日间平均舒张压(dDBP)、夜间平均收缩压(nSBP)、夜间平均舒张压(nDBP)分别增加了2.10、1.96、2.07、1.92、1.93、1.71 mm Hg(均P<0.05)。分层分析结果示:在高血压人群中,以低颈围组为对照,高颈围组24hSBP、24hDBP、dSBP、dDBP、nSBP、nDBP分别增加了2.66、1.97、2.57、1.92、2.64、1.82 mm Hg(均P<0.05)。在非高血压人群中,低颈围组与高颈围组24hABPM参数差异无统计学意义(均P>0.05)。结论 高血压患者中,颈围与24hSBP、24hDBP、dSBP、dDBP、nSBP、nDBP均呈正向关联。Objective To investigate the association between neck circumference and 24 h ambulatory blood pressure monitoring(24 hABPM) parameters. Methods Participants from Kailuan study, who completed medical information and received 24 hABPM during 2014-2019 were selected. They were divided into three groups according to the tertile of neck circumference of different gender. Generalized linear model was used to analyze the association between neck circumference and 24 hABPM parameters in different groups. Stratified analysis was conducted according to whether the participants had hypertension or not. Results A total of 2 504 participants met the inclusion criteria, with an average age of(63.0±10.8) years and a mean neck circumference of(38.1±3.6) cm, of which 1 709(68.2%) were males. The generalized linear models indicated that 24 hour mean systolic blood pressure(24 hSBP), 24 hour mean diastolic blood pressure(24 hDBP), daytime mean systolic blood pressure(dSBP), daytime mean diastolic blood pressure(dDBP), nighttime mean systolic blood pressure(nSBP), nighttime mean diastolic blood pressure(nDBP) in high neck circumference group increased by 2.10, 1.96, 2.07, 1.92, 1.93, and 1.71 mm Hg respectively as compared to the low neck circumference group(all P<0.05), after adjusting for age, gender, smoking, alcohol consumption, education, total cholesterol, high-sensitivity C-reactive protein, antihypertensive medication, snoring, history of diabetes, physical activity and body mass index(BMI). Stratified analysis showed that 24 hSBP, 24 hDBP, dSBP, dDBP, nSBP, and nDBP in high neck circumference group increased by 2.66, 1.97, 2.57, 1.92, 2.64, and 1.82 mm Hg, respectively, as compared to the low neck circumference group in hypertensives(all P<0.05), while in normotensives, the 24 hABPM parameters had no significant difference between high and low neck circumference groups(P>0.05). Conclusion In hypertensive patients, neck circumference is positively associated with 24 hSBP, 24 hDBP, dSBP, dDBP, nSBP, and nDBP.
