曹建雄教授运用黄芪防风加味汤治疗肺癌合并上腔静脉阻塞综合征临床经验

摘　　要：上腔静脉阻塞综合征是肺癌并发症之一,曹建雄教授认为,肺癌合并上腔静脉阻塞综合征常以咳嗽伴面颈部水肿、胸壁静脉曲张为典型表现,病机多为肺脾两虚,痰湿蕴结,兼肺卫不固,水湿瘀血泛溢肌表所致。治疗上,遵循“攻补兼施,表里分消”治则,以益气健脾补肺,祛风利水,活血化瘀为主,兼化痰散结。黄芪防风加味汤是曹建雄教授以黄芪防风汤加茯苓、浙贝母、麸炒海螵蛸、泽泻、益母草、怀牛膝、牵牛子而成的,运用于肺癌合并上腔静脉阻塞综合征中取得了一定的疗效。Superior vena cava occlusion syndrome is one of the complications of lung cancer.Professor CAO Jianxiong believes that lung cancer plus superior vena cava occlusion syndrome is often characterized by cough,facial and neck edema,and varicose veins in the chest wall.The pathogenesis is mostly lung and spleen both deficiency,accumulation of phlegm-dampness,accompanied by insufficiency of lung resistaance,and water-dampness and blood stasis overflowed in muscle surface.In terms of treatment,the principle of“combining elimination and reinforcement,and dispelling external and internal pathegens”is emphasized in clinical practice.The treatment is based on invigorating Qi(气),spleen,and lung,dispelling wind and diuresis,and promoting blood circulation to remove blood stasis,accompanied by resolving phlegm and dissipating mass.The Huangqi Fangfeng Jiawei decoction(黄芪防风加味汤)comes from Prof.CAO Jianxiong,is composed of the Huangqi Fangfeng decoction(黄芪防风汤)plus Fuling(Poria cocos),Zhebeimu(Fritillaria),Fuchao Haipiaoxiao(Bran fried sepium),Zexie(Alisma),Yimucao(Motherwort),Huainiuxi(Achyranthes),Qianniuzi(Morning glory seed)and so on,and shows certain curative effect on superior vena cava occlusion syndrome.
