摘  要:目的探讨MRI常规序列检查在边缘系统脑炎(LE)和边缘系统胶质瘤(LG)鉴别诊断中的价值.方法回顾性分析20例LE和45例LG患者的MRI常规序列的图像特征.结果单变量分析结果显示病变邻近大脑中动脉水平段走行区外侧裂池受压移位或变形、占位效应、病变周围水肿和病变累及边缘系统以外结构、单侧病变与LG关系密切;病变邻近皮质下灰白质界限存在与LE相关性高.多因素Logistic回归分析结果显示大脑中动脉水平段走行区外侧裂池受压移位或变形和单侧病变是LG的重要识别参数[优势比(OR=11.869,OR=0.299)],而病变邻近皮质下灰白质界限存在是LE的重要相关参数(OR=9.951).结论MRI常规序列的大脑中动脉水平段走行区外侧裂池形态、病变侧别、邻近皮质下灰白质界限征象有助于鉴别LE和LG并指导临床治疗.Objective To explore the value of MRI conventional sequences examination in the differential diagnosis between limbic system encephalitis(LE)and limbic system glioma(LG).Methods The images characteristics of MRI conventional sequences in 20 cases of LE and 45 cases of LG were analyzed retrospectively.Results Univariate analysis showed a close relationship between compression displacement or deformation of the lateral fissure in the walking area of the horizontal segment of the middle cerebral artery adjacent to the lesion,mass effect,peripathic edema,and lesion involvement of structures other than the limbic system,unilateral lesions,and LG.The presence of lesions adjacent subcortical gray matter boundary was highly correlated with LE.Multivariate Logistic regression analysis showed that compression displacement or deformation of the lateral fissure in the walking area of the horizontal segment of the middle cerebral artery and unilateral lesions were important parameters for identifying LG[odds ratio(OR)=11.869,OR=0.299],while the presence of lesions adjacent subcortical gray matter boundary was important relevant parameter for LE(OR=9.951).ConclusionMRI conventional sequences shows the morphology of lateral fissure in the walking area of the horizontal segment of the middle cerebral artery,the side of the lesion and the signs of adjacent subcortical gray matter boundary are helpful to distinguish LE fromLGandguideclinical treatment.
