摘　　要：目的探讨新生儿卵巢囊肿蒂扭转的CT特点.方法选取经手术证实为卵巢囊肿的患儿66例,根据是否合并蒂扭转分为2组,回顾性分析2组病灶CT表现(囊壁钙化、囊内分隔、液平面、扭转蒂)并对其进行统计学分析.结果66例患儿,64例为单侧囊肿,2例为双侧囊肿,共发现68个卵巢囊肿.35个卵巢囊肿合并蒂扭转(其中7个合并卵巢自截),33个为单纯卵巢囊肿.35个卵巢囊肿蒂扭转病灶中,囊壁钙化25例(71.4%),囊内分隔20例(57.1%),囊内分隔并钙化13例(37.1%),液平面16例(45.7%),扭转蒂29例(82.6%),扭转蒂伴钙化19例(54.3%).33个单纯卵巢囊肿病灶中,囊壁钙化1例(0.3%),囊内分隔1例(0.3%),囊内分隔并钙化1例(0.3%).2组病灶CT征象差异均有统计学意义(P<0.001).结论新生儿卵巢囊肿蒂扭转的CT直接征象为扭转蒂,CT表现为复杂型囊肿高度提示卵巢囊肿蒂扭转.Objective To investigate the CT manifestations of ovarian cysts complicated with torsion in infants.Methods A total of 66 infants with ovarian cysts confirmed by surgery were selected.Sixty-six infants were divided into two groups according to whether they complicated with torsion.The CT findings(cyst wall calcification,intracapsular separation,fluid level,twisted pedicle)of the lesions in the two groups were retrospectively analyzed and statistically analyzed.Results Sixty-eight ovarian cysts were found in 66 infants,64 cases were unilateral cysts and 2 cases were bilateral cysts.There were 35 ovarian cysts complicated with torsion(7 of them with ovarian autoamputation)and 33 with simple ovarian cysts.Among the 35 ovarian cysts complicated with torsion,there were 25 cases(71.4%)of cyst wall calcification,20 cases(57.1%)of intracapsular separation,13 cases(37.1%)of intracapsular separation with calcification,16 cases(45.7%)of fluid level,29 cases(82.6%)of twisted pedicle,and 19 cases(54.3%)of twisted pedicle with calcification.Among the 33 simple ovarian cyst lesions,there was calcification in the cyst wall in 1 case(o.3%),intracapsular separation in 1 case(0.3%),and intracapsular separation with calcification in 1 case(0.3%).There were statistically significant differences in CT findings between the two groups(P<0.001).Conclusion The direct CT findings of ovarian cysts complicated with torsion in infants are twisted pedicles,CT findings of complex cysts are highly suggestive of ovarian cysts complicated withtorsion.
